Does Higher Gadolinium Concentration Play a Role in the Morphologic Assessment of Brain Tumors? Results of a Multicenter Intraindividual Crossover Comparison of Gadobutrol versus Gadobenate Dimeglumine (the MERIT Study)

Abstract: BACKGROUND AND PURPOSE:Gadobenate dimeglumine has proved advantageous compared with other gadolinium-based contrast agents for contrast-enhanced brain MR imaging. Gadobutrol is a more highly concentrated agent (1.0 mol/L). This study intraindividually compared 0.1-mmol/kg doses of these agents for qualitative and quantitative evaluation of brain tumors.

“…Comparative studies in other indications have similarly highlighted the value of high-relaxivity gadobenate dimeglumine in permitting the use of lower doses. 21,22 As noted elsewhere, [23][24][25] the higher in vivo R1 relaxivity of gadobenate dimeglumine is due to weak, transient interaction of the gadobenate contrast- effective molecule with serum albumin, 26 which slows its molecular tumbling rate, leading to greater shortening of the T1 relaxation time and thus substantially increased signal intensity enhancement.…”

“…[1][2][3] Many of the included studies, however, used either a double dose of gadolinium contrast agent or a standard volume of [25][26][27][28][29][30] mL (corresponding to 0.17-0.2 mmol/kg for a 75-kg patient). Although some studies have used lower doses, this use has often been associated with compromised spatial resolution and image quality.…”

“…12 Thus, large-scale, well-controlled, prospective clinical studies have found minimal differences between GBCAs with similar r1 relaxivity (eg, gadopentetate dimeglumine versus gadoteridol , and substantially improved SI enhancement and imaging performance for GBCAs with markedly higher r1 relaxivity (ie, gadobenate versus gadopentetate, 1,2 gadobenate versus gadodiamide, 3 gadobenate versus gadobutrol 4 ). Our results fully support these previous findings in confirming significantly superior imaging performance for gadobenate over gadoterate for both qualitative and quantitative enhancement when these agents are administered at equivalent approved doses of 0.1 mmol/kg body weight.…”

“…5 That differences in r1 relaxivity reflect differences in diagnostic efficacy is borne out by evidence from numerous large-scale intraindividual crossover studies. [1][2][3][4][8][9][10][11] These studies have shown that differences in SI enhancement and imaging performance reflect differences in r1 relaxivity and that the magnitude of the differences in SI enhancement a Comparisons based on the number of patients with both postdose T1SE images assessed and technically adequate: n ϭ 96 for reader 1, n ϭ 94 for reader 2, and n ϭ 95 for reader 3.
