Does General Anesthesia Increase the Diagnostic Yield of Endoscopic Ultrasound-guided Fine Needle Aspiration of Pancreatic Masses?

Abstract: : Anesthesiologist-delivered GA was associated with a significantly higher diagnostic yield of EUS-FNA. GA should be considered a preferred sedation method for EUS-FNA of a solid pancreatic mass.

“…Because cost-effectiveness of sedation with propofol has not been established, the American and European Society of Gastroenterology do not take a stand on this subject 11 13 . Although we did not ask participants for the reasons behind their choice, previous studies have suggested that the increased use of propofol in the United States is caused by: 1) the believe that it improves the diagnostic accuracy of EUS-guided tissue sampling; 2) efforts to offset falling procedure reimbursements; and 3) marketing strategies of anesthesiologists 13 21 22 .…”

“…The first task in performing high quality EUS-FNA involves locating the target tissue and determining the ideal needle approach. Perhaps for this reason, use of general anesthesia has shown to be associated with increased diagnostic yield (83% with vs 73% without) during EUS-FNA of pancreatic mass lesions[ 21 ]. However this was a single center retrospective study and further study is required to confirm these results.…”

“…In a retrospective study on 371 patients it was shown that using general anesthesia (either with or without intubation) increased significantly the rate of cytological diagnosis compared to conscious sedation. [20] Finally, in cases with a high suspicion of cancer and previous inconclusive or negative FNA findings, studies have shown a benefit in increasing final number of true positive results when procedure is repeated. [2122]…”
