Selenium deficiency constitutes a risk factor for the incidence and negative course of severe diseases including sepsis, stroke, autoimmune diseases or cancer. In this study, hypoxia is identified as a powerful stimulus to redirect selenoprotein biosynthesis causing reduced selenoprotein P expression and diminished selenium export from hepatocytes in favour of increased biosynthesis of the essential protective intracellular phospholipid hydroperoxide glutathione peroxidase GPX4. Specifically, hypoxia decreases transcript concentrations of central factors controlling selenium and selenocysteine metabolism including selenophosphate synthetase-2, phosphoseryl-tRNA(SerSec) kinase and selenocysteine lyase, which are all proven to be rate-limiting enzymes in selenoprotein biosynthesis. These effects are paralleled by a general decline of selenoprotein expression; however, not all selenoproteins are affected to the same extent by hypoxia, and GPX4 constitutes an exception as its expression becomes slightly increased. Supplemental selenium is able to overcome the hypoxia-dependent down regulation of selenoprotein expression in our cell culture model system, supporting the concept of using selenium as an adjuvant treatment option in severe diseases. Although it remains to be tested whether these effects constitute a hepatocyte-specific response, the selenium-dependent decline of selenoprotein P biosynthesis under hypoxic conditions may explain the progressive selenium deficit developing in severe diseases.

The annual congress of the European Society of Cardiology (ESC) was held in Vienna, Austria, September 1 to 5, 2007. The total attendance was close to 30,000 participants from 136 different countries. Excellent new congress facilities hosted 186 prearranged sessions (28 meeting rooms running in parallel), including several joint sessions organized in collaboration with other societies, including the American College of Cardiology (ACC) and the American Heart Association (AHA). A total of 9,691 abstracts from 92 different countries were submitted, and 3,501 (36%) were selected for presentation, including 29% dedicated to basic science.The theme of the meeting was "Heart Failure." The pathophysiology, diagnosis, and management of patients with heart failure (HF) were addressed in 37 pre-arranged sessions and 16 scientific abstracts sessions. In this review, the contributions related to HF are discussed first, followed by the Hotlines and the Euro Heart Survey Program. Thereafter, a summary of the most important contributions presented at the different sessions is provided. Heart FailureHeart failure is a major cause of hospital stays in the Western world, and many patients are readmitted or suffer from high mortality after discharge. A large registry followed 48,612 patients with acute HF after discharge (1). A pre-specified sample of 10% was followed for 60 to 90 days and investigated for coronary revascularization. Patients with ischemic etiology of HF had a significantly worse outcome at 90 days as compared with nonischemic HF, although patients with ischemic HF who underwent coronary revascularization had similar survival to patients with nonischemic HF. These observations underscore that evaluation of coronary status in patients admitted to the hospital with acute HF is crucial.A pilot study investigated the effects of chronic use of sildenafil on exercise capacity and reactive pulmonary hypertension in patients with HF in a controlled clinical trial, enrolling 19 HF patients with New York Heart Association class I to III function, with left ventricular ejection fraction (LVEF) Յ40%, and who were on optimized medical therapy (2). Peak oxygen uptake increased (ϩ13%) and pulmonary pressure decreased (Ϫ32%) significantly in the sildenafil group.

Проанализирована роль селена в многофакторном этиопатогенезе ишемического инсуль-та. В клинических условиях у пациентов с диагнозом «ишемический инсульт» определены показатели биомаркеров дисциркуляторной патологии. Методом атомно-эмиссионной спектрометрии с индуктивно связанной аргоновой плазмой определена концентрация селена в сыворотке крови. Методом иммуно-ферментного анализа в сыворотке крови определена концентрация нейроспецифического белка AT к NR2 (нг/мл), являющегося строгим предиктором ишемического инсульта и характеризующего степень повреждения ишемизированной мозговой ткани. Неинвазивным нейрофизиологическим методом про-ведена регистрация уровня постоянных потенциалов головного мозга (УПП, мВ) в разных возрастных группах. Методом Манна−Уитни показано достоверно более низкое значение концентрации селена в старшей возрастной группе (больше 60 лет), что свидетельствует об истощении глутатионовых антиок-сидантных ферментов в позднем возрасте. Выявлена сопряженность концентраций селена и AT к NR2: более высокому значению селена соответствует более низкая концентрация AT к NR2 и наоборот. Дан-ный факт подтверждает взаимозависимость антиоксидантной защиты, показателем которой является селен, и нейроспецифического белка AT к NR2 − показателя повреждения ишемизированной мозговой ткани. Корреляционный анализ выявил достоверную связь между концентрацией селена и УПП в раз-личных областях мозга и между концентрациями селена и AT к NR2, что свидетельствует о возможно-сти использования этих показателей в качестве биомаркеров ишемии мозга с целью обеспечения мак-симальной сохранности мозговой ткани при дисциркуляционных расстройствах.КЛЮЧЕВЫЕ СЛОВА: ишемический инсульт, селен, AT к NR2, энергетический метаболизм мозга, уровень постоянного потенциала, макроэлементы, микроэлементы.
