Sudden cardiac arrest remains a relevant problem with a significant number of deaths worldwide. Although survival rates have more than tripled over the last 20 years (4% in 2001 vs. 14% in 2020), survival rates with good neurological outcomes remain persistently low, representing a major socioeconomic problem. Every minute of delay from patient collapse to start cardiopulmonary resuscitation (CPR) and early defibrillation reduces the chance of survival by approximately 10-12%. Therefore, the time to treatment is a crucial factor in the prognosis of patients with out-of-hospital cardiac arrest (OHCA). Research teams working in the pre-hospital setting are therefore looking for ways to improve the transmission of information from the site of an emergency event and to make it easier for emergency medical dispatch centres (EMDC) to recognise life-threatening conditions with minimal deviation. For emergency unit procedures already at the scene of the event, methods are being sought to efficiently and temporarily replace a non-functioning cardiopulmonary system. In the case of traumatic cardiac arrest (TCA), the focus is mainly on effective affecting noncompressible haemorrhage.

ZusammenfassungResuscitative Endovascular Balloon Occlusion of the Aorta (REBOA) ist ein wenig invasives Verfahren, um ein „internes“ Cross-Clamping der Aorta durchzuführen. Hierdurch ist es entweder möglich, den Blutverlust bei nicht komprimierbaren Körperstammblutungen zu reduzieren und den Patienten vor dem Verbluten zu retten oder im Rahmen der Herz-Lungen-Wiederbelebung (CPR) den koronaren und zerebralen Perfusionsdruck zu erhöhen und damit eventuell die ROSC-Rate und das neurologische Outcome zu verbessern. Aktuell findet REBOA in Deutschland nur in einigen Traumazentren im Schockraum Anwendung und der prähospitale Einsatz ist eher eine extrem seltene Ausnahme. Wir berichten über 2 Fälle, in denen das Verfahren im Rahmen des RIBCAP-HEMS-Projektes (REBOA In Bleeding and Cardiac Arrest in the Pre-hospital care by Helicopter Emergency Medical Service) prähospital durch das Team eines Rettungshubschraubers der DRF Luftrettung jeweils bei der CPR sowie beim Trauma angewendet wurde.

REUE | EditorialEl 8 de junio de 1923 toreó en Madrid Chicuelo, el cual no consiguió un desquite completo en esa cuarta corrida en la corte, lo que hizo no le contrataran en provincias por sus "éxitos". En el Senado a las cuatro menos cuarto, el Conde de Romanones abrió la sesión, en la que el Conde de Mortera habló sobre el problema de Marruecos; decía que "se equivocó el objeto, y en vez de un protectorado se había hecho una ocupación militar, que condenaban el pueblo y los políticos" 1 .Al mismo tiempo, en el Hospital de Dar Drius, en Marruecos, habían ingresado varios militares heridos muy graves, entre otros, el teniente Rafael Carbonell, el soldado Agustín Vega; grave el soldado José Mora, y menos grave el soldado Abdón Martín, entre otros... 2 . Las autoridades militares sanitarias de ese hospital deciden que, para salvar la vida de algunas de las víctimas debían trasladarlas a Nador, donde había más recursos asistenciales. No era mucha la distancia, pero por tierra se tardarían con las caballerías dos días, lo que supondría la muerte de esos pacientes. Por ello, se decide hacerlo por aire, en un avión Breguet XIV, avión biplano de madera, lona y metal [3][4][5] .El teniente Rafael Carbonell Muñoz, se convirtió en el primer herido que se trasladaba en avión en España. Nunca había volado; es trasladado por compañeros en una camilla hasta el avión, tiene menos dolor por la morfina que le han puesto, la cual también lo mantiene tranquilo. Es introducido en el avión que huele a combustible; algo en él se tensa al oír el rugir del motor Fiat de 300 CV. El sanitario que le acompaña, el cabo Fullá, le da la mano. Los dos se miran, cierran los ojos, notan como el estómago se mueve y el teniente Sbarbí, piloto de aquel vuelo, les dice que ya están en el aire, 25 minutos después toman tierra en el aeródromo de Nador, donde le espera una ambulancia para llevarle al hospital. Mientras, en Dar Drius se corre la noticia entre la tropa: han trasladado a un herido en avión para poderlo curar. Los soldados sonríen, se sienten más seguros.
