Do reflex seizures and spontaneous seizures form a continuum? – Triggering factors and possible common mechanisms

Abstract: Recent changes in the understanding and classification of reflex seizures have fuelled a debate on triggering mechanisms of seizures and their conceptual organization. Previous studies and patient reports have listed extrinsic and intrinsic triggers, albeit their multifactorial and dynamic nature is poorly understood. This paper aims to review literature on extrinsic and intrinsic seizure triggers and to discuss common mechanisms among them. Among self-reported seizure triggers, emotional stress is most freque… Show more

“…Some precipitating factors, such as sleep deprivation, are common, affecting many epilepsy patients, whereas other precipitants are more specific, such as photostimulation . The mechanisms by which precipitating factors lower the seizure threshold are most likely quite diverse …”

“…Age‐adjusted prevalence estimates for epilepsy range from 2.7 to 17.6 per 1,000 population . Although several external and internal trigger factors for epileptic seizures have been identified including sleep deprivation, sensory stimulation, psychological stress, and hyperventilation, most seizures occur unexpectedly and unpredictably and are independent of these factors . However, patients report changes in weather as being one of the most significant risk factor for seizures .…”

“…Spontaneous and reflex seizures have similar patterns [111] and are supposed to represent the two extremities of a continuum on which seizures are generated by extrinsic and intrinsic triggers [112]. Thus, kindling produced by reflex seizures may be a valuable tool for studying basic mechanisms of secondary epileptogenesis, clarifying pathways of seizure generalization in the brain, and searching for approaches to pharmacological modulation of epileptogenesis.…”
