The aim of this study was to evaluate nasal airway resistance and nasal mucociliary activity in Familial Mediterranean Fever (FMF) patients using anterior rhinomanometry and saccharin clearance test. Methods: 30 patients with FMF disease and 30 healthy individuals were included in this prospective, cross-sectional study. Nasal airway resistance was measured via active anterior rhinomanometry and saccharin test was carried out for the measurement of nasal mucociliary clearance (NMC) times of all participants. Results: Mean values for NMC time in the FMF patient group and control group were found as 14.6 ± 4.7 (range, 8-30) min. and 9.7 ± 2.3 (range, 6-14) min. respectively. Nasal mucociliary clearance time in the FMF patients was significantly prolonged in comparison that of healthy controls (p < 0.001). We identified no significant correlation between the disease duration and NMC time (p=0.921, r = 0.019). When the mean nasal airway resistance values of both groups were compared, there was no significant difference was detected (p = 0.371). Conclusions:The results of our study have revealed that NMC time was longer in patients with FMF than that healthy controls. However, there was no statistically significant correlation between nasal mucociliary clearance time and disease duration. Disruption of NMC increases the risk of infection in the upper and lower respiratory tract. Caution should be exercised against upper and lower respiratory infections in patients with FMF.

Purpose The aim of this study was to reveal the effect of N95 and surgical masks on mucociliary clearance function and sinonasal complaints. Methods Sixty participants were enrolled in this study, including 30 people in N95 mask group and 30 people in surgical mask group. Two interviews, three days apart, were performed with all participants. The participants were asked not to use any mask before the first interview while they were asked to use the determined mask just before the second interview for 8 h. In both interviews, the mucociliary clearance times (MCTs) were measured and participants were asked to score ten distinct sinonasal complaints using visual analog scale (VAS). Data obtained from first interview were named pre-mask data, data obtained from second interview were called after-mask data. In both groups, pre-mask MCTs and VAS scores were compared with after-mask MCTs and VAS scores. Results After-mask MCTs (mean = 13.03 ± 6.05 min) were significantly longer than pre-mask MCTs (mean = 10.19 ± 4.21 min) in N95 mask group ( p = 0.002). No significant difference was found between after-mask and pre-mask MCTs (mean = 12.05 ± 5.21 min, mean = 11.00 ± 5.44 min, respectively) in surgical mask group ( p = 0.234). When after-mask VAS scores were compared with pre-mask VAS scores, it was found that N95 mask use increased nasal blockage and postnasal discharge, surgical mask usage increased nasal blockage. Conclusion While the use of N95 mask leads to nasal blockage and postnasal discharge, surgical mask use results in nasal blockage. N95 masks cause impairment in mucociliary clearance function. But all these effects are mild. Surgical masks have not been found to have any effect on mucociliary clearance function.

Background: Functional endoscopic sinus surgery is widely practiced during daily otorhinolaryngology practice. It usually done under low blood pressure. However, the ideal hypotensive drug or technique is yet not well established. Aim of the work: To evaluate outcome of functional endoscopic sinus surgery under esmolol-induced hypotensive anesthesia [EHA] versus hypotensive total intravenous anesthesia [H-TIVA]. Patients & Methods: 72 patients were randomly divided into two groups; Group A: H-TIVA and Group B [EHA]. After induction of anesthesia, remifentanil/ propofol and esmolol infusions were adjusted to maintain mean arterial pressure at 60-70 mmHg. After completion of surgical procedure, esmolol infusion was stopped, while remifentanil/propofol infusions were adjusted to allow restoration of blood pressure. Operative field bleeding and visibility were graded using Fromme scale. Primary outcome is the efficacy of EHA to minimize intraoperative bleeding to an extent that allows satisfactory completion of surgery. Results: Esmolol bolus significantly attenuated pressor reflexes to induction and intubation than induction by remifentanil. Intraoperative [IO] heart rate and MAP measures were significantly lower with EHA. At 10-minures after infusion stoppage, patients of group B still had significantly lower HR and MAP, while at 10-min later, the difference was nonsignificant. HA minimized IO blood loss down to no to slight bleeding in 16.7% and 55.5% of studied patients and improved field visibility to satisfactory-to-good levels in 51.4% and 43.1% of surgeries, respectively. Moreover, EHA provided better field visibility, so allowed significant reduction of operative time than with TIVA. Conclusion: Hypotensive anesthesia is safe and appropriate modality for FESS and improves surgical and clinical outcome. Both esmolol and remifentanil provided satisfactory results. Esmolol is superior to remifentanil.
