For patients being discharged from the hospital, this may mean more efficient use of postacute care (PAC), including discharging patients to higher quality skilled nursing facilities (SNFs) in an effort to limit readmissions and other costly complications. Public reporting of nursing home quality has been associated with improved performance measures, although improvements in preventable hospitalizations have la… Show more

“…Under this payment model, quality tended to improve and, if not, to remain stable [40,[58][59][60][61][62][63][64][65][66][67][68][69][70][71][72][73][74][75]. The same was true for spending [59-61, 66-68, 72-89], whereas the effects on utilization were more diverse [41, 58-61, 63, 67, 68, 72, 74-76, 82-84, 86, 88, 90-100].…”

“…Some ACOs have created preferred SNF networks, gently steering patients to providers where they may have overlapping staff, shared trainings, or a bi-directional flow of information. There is evidence of positive steering 4 , 5 and decreased readmissions rates within ACOs. 6 At the Institute for Accountable Care (IAC), we have noted that the mean SNF stay costs for ACO beneficiaries in 2021 was $16,362, compared with $20,954 for fee-for-service beneficiaries, further hinting at some form of positive steering to efficient facilities.…”
