Diverticulum of Kommerell: A Review of a Series and a Report of a Case with Tracheal Deviation Compromising Single Lung Ventilation

Abstract: Birth defects can affect the great vessels of the aorta and its branches, compressing the upper airway and the swallowing track. Surgical repair of these defects requires manipulating the airway to improve surgical exposure. This is a report of a series of cases with compromised airway resulting from such a birth defect and methods of its management.

“…There are two variants that are commonly described in literature. Left-sided aortic arch with aberrant right subclavian artery (ARSA prevalence 0.5-2.0%) or right aortic arch with aberrant left subclavian artery (prevalence 0.05-0.1%) [3]. Kommerell diverticulum occurs in a number of anomalies of aortic arch system.…”

“…Most of the patients with KD are asymptomatic, found incidentally on imaging studies of the chest. Common symptoms of KD include dysphagia, cough, chest pain and stridor due to extrinsic compression of esophagus or tracheobronchial tree by the KD or dilatation of an aberrant subclavian artery [1,3]. Dysphagia is the most common symptom with KD, termed dysphagia lusoria [6].…”

“…Some authors advocate that all Kommerell's diverticulum should be surgically addressed regardless of size to prevent rupture [8]. Other surgeons recommend interventions for symptomatic diverticuli larger than 5 cm [4].…”

“…Diese kongenitale Aussackung der Aorta ist definiert als aneurysmatisch ausgeweiteter Abgang einer aberranten rechten A. subclavia (bei linkem Aortenbogen; 0,5% der Bevölkerung) bzw. aberranten linken A. subclavia (bei rechtem Aortenbogen; 0,1% der Bevölkerung) [21]. Entwicklungsgeschichtlich handelt es sich hier um das persistierende Ende des unterbrochenen 4.…”
