AIM:To study the clinical outcomes of medical therapy in patients with right colonic diverticulitis. METHODS:The records of 189 patients with right colonic diverticulitis which was finally diagnosed by computed tomography, ultrasonography, or operative findings were retrospectively reviewed. RESULTS:Of the 189 patients hospitalized for right colonic diverticulitis, the stages of diverticulitis by a modified Hinchey classification were 26 patients (13.8%) in stage 0, 139 patients (73.5%) in stage Ⅰ a, 23 patients (12.2%) in stage Ⅰb, and 1 patient (0.5%) in stage Ⅲ. Medical therapy was undertaken in 185 of 189 patients (97.9%). One hundred and eighty three of 185 patients were successfully treated with bowel rest and antibiotics. Two patients in stage Ⅰb required a resection or surgical drainage because of an inadequate response to conservative treatment. Recurrent diverticulitis developed in 15 of 183 patients (8.2%) who responded to medical therapy. All 15 patients who suffered a second attack had uncomplicated diverticulitis, and were successfully treated with medical therapy. CONCLUSION:Our results indicate that right colonic diverticulitis is essentially benign and image-guided conservative treatment is primarily required.

Colonic hepatic flexure diverticulitisObjective: Right colon diverticulitis is a rare entity in our means. Frequently, this entity is mistaken for other abdominal conditions, usually appendicitis, right colon cancer or acute cholecystitis. We found no specific reports about their location in the hepatic flexure of the colon. Material and Method: 58-years-old male who consulted the emergency department with abdominal pain and palpable mass in right upper quadrant. Abdominal CT revealed diverticulitis of the hepatic flexure of the colon. Results: Successful outcome with medical treatment. The patient is asymptomatic at 3 months of follow. Conclusion: Diverticulitis of the hepatic flexure of the colon is a rare entity that can mimic other diseases and usually responds to conservative treatment.Key words: Diverticulitis, right colon, medical treatment. ResumenIntroducción: La diverticulitis del colon derecho es una entidad poco frecuente en nuestro medio. Con relativa frecuencia, esta entidad es confundida con otros procesos abdominales, habitualmente apendicitis, cáncer de colon derecho o colecistitis aguda. No encontramos reportes específicos sobre su localización en la flexura hepática del colon. Material y Método: Varón 58 años quien consultó en urgencias por dolor abdominal y masa palpable en hipocondrio derecho. Se realiza TC abdominal que revela diverticulitis del ángulo hepático del colon. Resultados: buena evolución con tratamiento médico; el paciente se encuentra asintomático a tres meses del alta. Conclusión: La diverticulitis del ángulo hepático del colon es una entidad poco habitual, que puede simular otras patologías y que habitualmente responde al tratamiento conservador.Palabras clave: Diverticulitis, colon derecho, tratamiento médico.

Aim: To evaluate the similarities and differences in disease courses between right-and left-sided diverticulitis in a Western population. Methods:All consecutive patients admitted in a Dutch regional hospital between 2004 and 2008 for conservative or operative management of clinically and radiologically confirmed acute right-and left-sided diverticulitis were analyzed retrospectively. Diverticulitis was clinically suspect if the patient presented with lower abdominal pain combined with at least one elevated inflammatory parameter (temperature, WBC, CRP or ESR). Diverticulitis was radiologically confirmed if at least US or CT demonstrated signs of acute right-or left-sided diverticulitis. Differences in incidence, patient characteristics, clinical presentation and disease course between the two diagnoses were analyzed. Results:The hospital system yielded 425 patients with a diverticulosis/diverticulitis discharge coding. 57% was excluded because these patients had another diagnosis demonstrated by imaging, besides diverticulosis (19%), had no imaging at all (16%), were elective or referred (7%), were not admitted (4%) or had other reasons (11%). A total of 183 admitted patients with both clinical and radiological confirmed acute colonic diverticulitis were included. The incidence of right-sided diverticulitis was 8%. Patients with right-sided diverticulitis were predominantly female (86% compared to 47% in leftsided diverticulitis, P = 0.05). Median CRP at presentation was lower in right sided diverticulitis, 30 compared to 71 mg/L (P = 0.001). No other differences in clinical presentation, like the severity of diverticulitis, and disease course, like the need for acute surgery, were found between right and left-sided diverticulitis.Conclusions: Disease course of acute right-sided diverticulitis in Caucasians is comparable to left-sided diverticulitis. Left-sided diverticulitis treatment guidelines may also apply for the right-sided diverticulitis in Caucasians.
