Distribution of Conjunctival HLA-DR Expression and the Pathogenesis of Damage in Early Dry Eyes

Abstract: The HLA-DR expression pattern in mild and moderate dry eyes appears to reflect disease progression. Overexpression of HLA-DR in mild dry eyes showing no vital staining suggests that inflammation may be a primary cause of ocular surface damage. These data support the use of immunomodulatory drugs in the treatment of dry eye disease.

“…Moreover, the impression cytology and brush cytology samples can also be used to perform immunohistochemical staining, enzyme-linked immunosorbent assay, flow cytometry, and mRNA expression analysis. 9,10,29,30 Therefore, they are also useful to monitor the pathologic progress in cGVHD-related dry eye disease and helpful for investigating the etiology of cGVHD-related dry eye disease.…”

“…HLA-DR is a MHC class II antigen whose expression by conjunctival epithelial cells has been shown to correlate with the severity of dry eye, as characterised by vital staining and Schirmer test [31,32,33]. Contact lens wearers with dry eye show greater expression of HLA-DR compared to non-dry eye wearers [34].…”

“…It is characterized by focal lymphocytic infiltration of these secretory glands (dacryoadenitis and sialoadenitis), hypergammaglobulinaemia and production of autoantibodies, including anti-SS-A/Ro, anti-SS-B/La and anti-muscarinic type-3 acetylcholine receptor (M3R) [1,2]. Immunopathological features of the conjunctiva in Sjögren's syndrome include increased expression of cytokines, chemokines and immune activation and adhesion molecules [3,4]. An increase in the CD4/ CD8 T cell ratio in the conjunctiva has also been observed [5].…”
