Distraction of facets with intraarticular spacers as treatment for lumbar canal stenosis: report on a preliminary experience with 21 cases

Abstract: Object The authors report their experience in treating 21 patients by using a novel form of treatment of lumbar degenerative disease that leads to canal stenosis. The surgery involved distraction of the facets using specially designed Goel intraarticular spacers and was aimed at arthrodesis of the spinal segment in a distracted position. The operation is based on the premise that subtle and longstanding facet instability, joint space reduction, and subsequent facet o… Show more

“…11,13 Our experience in dealing with and manually handling unstable atlantoaxial and subaxial spinal facets is more than 25 years long. “…The mentioned clinical article of a preliminary report on intra articular spacers as treatment for lumbar canal stenosis, published in 2013 [4], indicates the essential role of facet joints but does not necessarily contribute to the discussed issue in our biomechanical paper.…”

“…Our studies have emphasized the significance of facetal distraction and fixation for the entire spine, particularly of the craniovertebral junction. [1][2][3][4][5] Basilar invagination is significantly common in India and in the Indian subcontinent. Even in India, there is a disproportionately high incidence in north-western belt of the country.…”
