Aim:We previously reported that glycemic control deteriorated in patients receiving atorvastatin, which is useful for the treatment of hypercholesterolemia in patients with type 2 diabetes. Pitavastatin has a strong lipid-lowering effect, comparable to that of atorvastatin, but it is unknown whether pitavastatin has an adverse influence on glycemic control. The aim of this study was to examine. The effects of three different statins (pravastatin, atorvastatin, and pitavastatin) on blood glucose and HbA1c levels in diabetic patients. Methods: We retrospectively compared glycemic control between groups receiving atorvastatin (10 mg/day, group A, n 99), pravastatin (10 mg/day, group Pr, n 85), and pitavastatin (2 mg/day, group Pi, n 95) from the start of treatment until 3 months later. Patients were excluded if the dosage of their antidiabetic drugs was changed, if their drug therapy was altered within 3 months before starting statin therapy, or if events occurred that could affect glycemic control such as hospitalization. Results: The subjects available for analysis were 74 patients from group A, 71 patients from group Pr, and 74 patients from group Pi. Arbitrary blood glucose levels increased from 147 51 mg/dL (mean SD) to 176 69 mg/dL in group A, but only changed minimally from 136 31 to 134 32 mg/dL in group Pr and from 155 53 to 154 51 mg/dL in group Pi. HbA1c increased from 7.0 1.1% to 7.4 1.2% in group A, while it was 6.9 0.9% versus 6.9 1.0% in group Pr, and 7.3 1.0% versus 7.2 1.0% in group Pi. There was no correlation between LDL-C and HbA1c (the change from baseline to 3 months) in any of the groups. Conclusion: The glycemic parameters only increased significantly in group A, suggesting that pitavastatin and pravastatin did not have an adverse influence on glycemic control in type 2 diabetic patients. OBJECTIVETo investigate the relationship between adherence with statin therapy and the risk of developing diabetes. RESEARCH DESIGN AND METHODSThe cohort comprised 115,709 residents of the Italian Lombardy region who were newly treated with statins during 2003 and 2004. Patients were followed from the index prescription until 2010. During this period, patients who began therapy with an antidiabetic agent or were hospitalized for a main diagnosis of type 2 diabetes were identified (outcome). Adherence was measured by the proportion of days covered (PDC) with statins (exposure). A proportional hazards model was fitted to estimate hazard ratios (HRs) and 95% CIs for the exposure-outcome association, after adjusting for several covariates. A set of sensitivity analyses was performed to account for sources of systematic uncertainty. RESULTSDuring follow-up, 11,154 cohort members experienced the outcome. Compared with patients with very-low adherence (PDC <25%), those with low (26-50%), intermediate (51-75%), and high ( ‡75%) adherence to statin therapy had HRs (95% CIs)

L'incidence du diabète de type 2 (DT2) atteint des proportions épidémiques ces dernières années en raison du vieillissement de la population et du développement de l'obésité, elle-même secondaire à l'augmentation de la sédentarité et à une alimentation trop riche en graisses. L'insulinorésistance est un événement précoce dans la physiopathologie du DT2, mais qui demeure relativement stable au cours de l'évolution de la maladie. À l'inverse, la capacité insulinosécrétrice de la cellule  pancréatique diminue au cours du temps et contribue de façon majoritaire à la détérioration progressive de l'équilibre glycémique chez les diabétiques de type 2 [1]. Étant donné que la carence fonctionnelle de la cellule  est potentiellement réversible, il apparaît important de développer des thérapeutiques visant à freiner cette insuffisance [2]. Un des mécanismes moléculaires les mieux étudiés actuellement est la lipotoxicité bêta-pancréatique, qui contribue, via l'accumulation intracellulaire de lipides, à l'altération de l'insulinosécrétion et au développement de l'apoptose [3,4]. Les espèces lipidiques incriminées sont essentiellement les acides gras libres circulants (notamment le palmitate) et les triglycérides, qui favorisent la mort de la cellule  en induisant la production de céramides cytotoxiques [3]. Plus récemment, des données suggè-rent que le cholestérol pourrait également jouer un rôle majeur dans le contrôle de la fonction cellulaire . Nous discuterons en détail ici les différents liens potentiels entre le métabolisme du cholestérol et le contrôle de l'insulinosécrétion. Le cholestérol intracellulaire : un nouvel acteur de l'insulinosécrétionPlusieurs approches expérimentales ont permis d'étu-dier la conséquence des variations de cholestérol intracellulaire sur la sécrétion d'insuline.Rôle de la synthèse de cholestérol intracellulaire SREBP2 (sterol response element binding protein-2) est un facteur de transcription qui régule les concentrations du cholestérol intracellulaire. En réponse à une baisse du cholestérol intracellulaire, SREBP2 stimule la synthèse endogène de cholestérol ainsi que la capture des LDL (low density lipoproteins) en augmentant > Le diabète de type 2 est fréquemment associé à une dyslipidémie mixte, ainsi qu'à une lipotoxicité caractérisée par une augmentation du contenu en triglycérides dans plusieurs types cellulaires. Au niveau de la cellule -pancréatique, cette lipotoxicité conduit à un défaut d'insulinosécrétion et à une apoptose. Plus récemment, de nouvelles données suggèrent l'existence d'un lien entre le métabolisme du cholestérol et la fonction des cellules . Ainsi, l'accumulation de cholestérol dans les îlots pancréatiques altère la sécrétion d'insuline. Sur le plan moléculaire, le transporteur ABCA1 (ATP-binding cassette transporter, member 1) et le récepteur des LDL (low density lipoproteins) apparaissent comme des médiateurs majeurs du métabolisme du cholestérol dans l'îlot.
