Distilling the Evidence on Developmental Care

Abstract: In most studies, the inclusion of multiple interventions made the determination of the effect of any single intervention difficult. Although there is evidence of some benefit of developmental care interventions overall and no major harmful effects reported, there were a large number of outcomes for which no or conflicting effects were shown. The single trials that did show a significant effect of an intervention on a major clinical outcome were based on small sample sizes, and the findings often were not suppo… Show more

“…The developmental care focus was introduced in the 1980's, as a strategy to change NICU environmental conditions with a view to neonatal wellbeing and safety by reducing noise levels and lighting intensity, minimal handling, provision of longer rest periods, among others (2) . Noise control can represents a difficult task for nurses, as NICU noise mostly derives from care activities and professional conduct (3)(4) .…”

“…Convincing scientific evidence supporting these hypotheses is not available to date. 17 The objective of this research was to analyze one area of impact of the SFR NICU; the perceptions of NICU staff regarding the quality of work conditions. The method used was to compare staff workplace quality perceptions in a conventional OPBY NICU with those in a new SFR NICU.…”

“…Care modified in this manner was associated with improved short-term outcomes; 5,6 however, data regarding the long term benefits have been more difficult to document, as reported by Symington in a meta-analysis of outcomes related to developmental care. 7 Investigators have been unable to establish the specific link between developmental interventions in the NICU and a reduction in adverse long-term neurodevelopmental consequences. The work of Graven 8 makes a compelling argument that given the immature state of brain development of neonates less than 30 weeks gestation and the active process of neuronal migration, the unregulated NICU environment can be expected to have harmful consequences.…”
