既往のない妊娠初期の妊婦が水中毒による意識障害を呈した1例(Impaired consciousness caused by water intoxication in a previously healthy woman in the first trimester of pregnancy: a case report)

“…水中毒による低Na血症は精神疾患合併例に多い 12)が，既往のない妊娠初期の妊婦での報告は稀である。妊娠に伴う水中毒の発症例としては，妊娠後期から分娩期にかけてオキシトシン投与に伴う例 2)や子宮鏡検査に伴う例 4), 5)などの報告がある。妊娠と水中毒の関連性を分析するため，PubMedで「pregnancy，water intoxication」をキーワードに検索し，1963～2021年の期間で101件の論文が抽出された。オキシトシン投与例や子宮鏡検査後，医原性など飲水に寄与しない報告例や基礎疾患に精神疾患があるものを除外したところ，2件の論文 2), 3)を認めた（Table 2）。発症年齢は28～38歳で，高齢出産での発症は1例であった。報告例のほとんどが妊娠後期・分娩期であり，飲水量も1日に5～10L程度であった。以上のことから，精神疾患のない妊婦においても水中毒に伴う低Na血症を発症すると推察されるが，本症例のような妊娠初期の症例は非常に稀であった。…”
