Management of severely comminuted, complete articular tibial pilon fractures (Rüedi and Allgöwer type III) remains a challenge, with few treatment options providing good clinical outcomes. Twenty patients with severely comminuted tibial pilon fractures underwent primary ankle arthrodesis with a retrograde calcaneal nail and autogenous fibular bone graft. The fusion rate was 100% and the varus malunion rate was 10%. Fracture union occurred at a mean of 16 weeks (range, 13-18 weeks) postoperatively. Primary ankle arthrodesis is a successful method for treating highly comminuted tibial pilon fractures, having a low complication rate and a high satisfaction score. Background:Pilon fractures are associated with a high incidence of complications, yet there continues to be uncertainty regarding variables associated with adverse outcomes after pilon open reduction and internal fixation (ORIF). This study sought to characterize the rate of arthrodesis and identify risk factors that increase the likelihood of ankle arthrodesis in patients with ORIF pilon fractures. Methods:After institutional review board approval, a retrospective review was conducted to identify patients who underwent ORIF of pilon fractures at an ACS Level I trauma center from 2005 to 2014. Medical records and radiographs were reviewed retrospectively for patient demographics, comorbidities, fracture characteristics, associated injuries, mechanism of injury, and functional activity. Logistic regression analyses were performed to identify risk factors associated with arthrodesis. Results:282 pilon fractures (279 patients) met inclusion criteria. There were 182 men and 97 women with an average age of 43.8 ± 15.2 yr. Overall, 15 pilon fractures (5.3%) in six male patients and nine female patients were treated with an ankle arthrodesis an average of 1.73 ± 5.15 yr after their initial injury. Following multivariable logistic regression analysis, age greater than 40 yr, tobacco use, and nonunion were identified as independent predictors of arthrodesis. Conclusions:This study found a number of patient-related factors associated with an increased risk of arthrodesis after a pilon fracture was sustained. Multivariable logistical analysis identified age greater than 40 yr, tobacco use, and nonunion as independent predictors of arthrodesis following pilon fracture ORIF.

The ankle plays an important role in daily life under the functional point of view. It is essential to perform basic movements such as walking, running, etc. The ankle joint complex is a part of the body most frequently injured of the musculoskeletal system. Orthopedic fractures in this joint may be classified as low trauma and high energy, the latter trauma, usually found in traffic accidents and falls from great heights are the most disabling. In the general context often this type of fracture compromises neighboring joints due to misalignment of the loading axis of the involved segment, emerging pain and progressive functional disability. In this context, this study aimed to review the literature of studies that were willing to analyze the biomechanical causes of the functional outcome of fractures of the complex orthopedic foot/ankle trauma generated by high and low energy. For this, a search was made in electronic databases indexing scientific articles, using the keyword search previously created. For better understanding we divided the literature review on topics: classification of ankle fractures, surgical treatment, no surgical indications and contraindications in cases of surgery, types of surgery with open reduction and internal fixation (ORIF), external and hybrid; wounds and complications consequences: amputation, obesity, arthritis and osteoarthritis, early motion, range of motion, gait pathology. From this review study, it is necessary that other publications in the field of physical therapy related biomechanical results of ankle fractures are carried out, since most studies were found in articles reporting on the medical surgeries.

Abstract: External skeletal fixation and minimal internal fixation of distal tibial pilon fractures is a good method for treating all types of inta-articular pilon fractures. In fractures types B and C dynamic external skeletal fixation allows early mobility in the ankle joint.
