Distal Renal Tubular Acidosis Caused by Tacrolimus in a Systemic Lupus Erythematosus Patient: A Case Report

Abstract

Renal tubular acidosis (RTA) refers to a group of disorders involving transport defects in bicarbonate reabsorption or hydrogen excretion. Features like metabolic acidosis with a normal anion gap, neurological symptoms, and electrolyte imbalances indicate RTA. Kidney transplantation, cirrhosis, sickle cell anemia, medications, and autoimmune diseases, particularly Sjögren's syndrome and rheumatoid arthritis, are related to RTA. We encountered a rare case of a patient with systemic lupus erythematosus accompanied by RTA secondary to tacrolimus administration, who had muscle weakness and paralysis. Her symptoms improved after discontinuing tacrolimus and correcting the acidosis and potassium levels. Here, we report on this case and review the relevant literature. (Korean J Med 2015;89:478-481) 서 론 신세관산증(renal tubular acidosis, RTA)은 근위세관 및 원 위세관에서의 중탄산염 재흡수 및 수소 배출의 장애로 인해 초래되는 전신성 대사산증으로서, 혈장 음이온차이(anion gap) 가 정상인 고염소혈산증(hyperchloremic acidosis) 상태이며, 대 개 사구체 여과율은 정상이다[1]. 대부분 특발성으로 나타나나 약제, 신기능 이상이나 부갑 상선항진증과 같은 내분비 이상으로 인해 이차성으로 나타나 는 경우가 있으며 드물게 쇼그렌증후군이나 그레이브스병, 전 신홍반루푸스 등에서 동반되어 발생하기도 한다. 면역질환 에 의해 후천적으로 발생하는 경우는 일반적으로 알파-사이 세포(alpha-intercalated cell)가 파괴되어 원위신세관산증이 발 생한다[2]. 약제에 의한 경우는 암포테리신 B (amphotericin B) 나 리튬(lithium), 칼시뉴린억제제(calcineurin inhibitor) 등이 관 련이 있는 것으로 알려져 있다. 신세관산증에 동반된 저칼륨혈증은 경증의 근력 저하로부 터 전신무력감, 횡문근융해, 호흡곤란 및 심정지에 이르기까 -Hae Koo Kim, et al. Lupus with distal renal tubular acidosis --479 -지 다양한 증상을 일으키고 대사산증의 경우 주로 호흡기, 순 환기, 신경계의 증상이 나타나며 신경계 증상으로 기면, 혼미, 혼수와 같은 의식 변화가 발생할 수 있다[3]. 저자들은 15년 간 발작장애를 동반한 전신홍반루푸스를 앓아 온 환자가 반 복적이며 일시적인 근력 저하 및 하지 감각이상 등의 증상을 보여 발작장애의 악화로 생각하여 신경과적 약제를 조절하다 타크로리무스(tacrolimus)에 의한 신세관산증으로 진단한 예를 경험하였다. 타크로리무스에 의한 신세관산증은 국내에서는 아직 보고된 바가 없어 이를 문헌 고찰과 함께 보고하는 바 이다.
