Dissociation in the Excretion of Different Aldosterone Metabolites and Unmetabolized (‘Free’) Aldosterone in Hypertension

Abstract: 1. The determination of aldosterone-18-glucuronide (pH 1-labile aldosterone) was complemented by concomitant measurements of free urinary aldosterone and tetrahydroaldosterone in 307 patients, most of whom were hypertensive. In 38 cases (12.3%) the normal, aldosterone-18-glucuronide concentration was clinically misleading, but increased free aldosterone and/or tetrahydroaldosterone values suggested the presence of hyperaldosteronism, which in many of these cases was confirmed by elevated excretion of the possi… Show more

“…The sum of conjugated and free tetrahydroaldosterone and 21-deoxytetrahy droaldosterone was the best discriminator for the patients with primary aldosteronism. The diagnostic shortcomings of the urine aldoste rone-18-glucuronide value have also pre viously been reported [10][11][12],…”

“…It is primarily a liver metabolite, corresponding to around half of the aldosterone metabolites [67,87]. Measuring tetrahydroaldosterone in the urine involves hydrolysis of the glucuronide conjugate with a beta-glucuronidase and measurement by several methods, including radioimmunoassay, highperformance liquid chromatography (HPLC)-MS-MS [88][89][90][91][92][93]. Since most aldosterone is metabolized to 3α,5β-tetrahydroaldosterone, it has been proposed that it is a better indicator of aldosterone secretion; however, the metabolism of aldosterone to the 3α,5βtetrahydroaldosterone is affected by different disease states; in particular, it is decreased in congestive heart failure and cirrhosis of the liver [94,95].…”

“…Das ist verständlich, denn bei den meisten Hyperplasie-Patienten werden im Referenzbereich liegende Werte vorgefunden. gen wurden auch von anderen Autoren (7) gemacht und bestätigen frühere eigene Untersuchungen (5,6). Bei einem großen Anteil dieser Patienten war 18-OH-Corticosteron erhöht, insbesondere in der Adenom-Gruppe.…”
