Disseminated retinoblastoma successfully treated with myeloablative chemotherapy – implication for molecular detection of minimal residual disease

Abstract: Summary:A useful marker for detecting minimal residual disease (MRD) has not been established yet in retinoblastoma. We assessed neuroendocrine protein gene product 9.5 (PGP9.5) expression, one of the markers for detecting MRD in neuroblastoma, in a patient with disseminated retinoblastoma. A 3-year-old boy with disseminated retinoblastoma in multiple bones and marrow was referred to our hospital. He received intensive treatment and has maintained CR for 48 months following myeloablative chemotherapy with hema… Show more

“…These include: the proposed superiority of thiotepacontaining regimens and the use of post transplant radiotherapy to involved bulky sites that could not be addressed by this study and the previous reports. 5,6,[16][17][18]20,26 The current international multi-center trial (COG ARET 0321) will be a key instrument for responding these questions. 27 Metastatic retinoblastoma is now less frequent in South America, 28 probably because of earlier diagnosis.…”

“…[12][13][14][15][16][17][18][19][20] Namouni et al 14 conducted a study of HDC consisting of carboplatin, etoposide and cyclophosphamide (CARBOPEC) followed by autologous SCT in 25 patients, including 12 patients with distant metastases. Among eight children with bone and BM metastases, five survived between 11 and 70 months disease free, while three patients with CNS metastases relapsed in the CNS after HDC and died.…”

“…On the other hand, although our treatment regimen for patients with higher risk PRFs was very efficacious, it was not devoid of significant toxicity and some patients may be cured with enucleation alone. Thus, as investigated for other pediatric tumors, the evaluation of minimally disseminated disease at diagnosis by PCR techniques could potentially be a way of identifying those cases requiring postoperative treatment and this strategy is currently being piloted by our group to augment risk assignment 41.…”
