Disseminated Histoplasmosis with Oral Manifestation in an Immunocompetent Patient

Abstract: A case of disseminated histoplasmosis (DH) in a 60-year-old female patient is reported from Jaipur, Rajasthan, India. The patient presented with multiple papules on the skin surrounding the lips, face, torso, trunk, and back. She also complained of growth in the palate. Histoplasmosis was confirmed by biopsy and histopathology of skin and palatal lesions. This case report highlights the presenting features and occurrence of histoplasmosis in nonendemic region in India.

“…Human and animal infections are caused by inhalation of fungus in dust [1]. In India, Histoplasmosis is endemic in West Bengal and Gangetic delta [2]. In humans Histoplasmosis may occur in three form: (i) Primary acute pulmonary form, (ii) chronic pulmonary and (iii) disseminated form.…”

“…Infection is largely caused by inhalation of droppings from infected birds or bats. Histoplasma capsulatum mainly infects the lungs and passes asymptomatically to involve the skin and reticuloendothelial system [ 1 ]. In immunocompetent individuals without underlying structural lung disease, the infection is usually benign and asymptomatic.…”

“…Histoplasmosis is extremely rare in presumed immunocompetent patients who do not reside in endemic areas [ 1 ]. This is a case of a young male who presented with right middle lobe pneumonia, later diagnosed with histoplasmosis and immunoglobulin (IgM) deficiency.…”

“…To the best of our knowledge, the literature contains only 1 report of an immunocompetent patient presenting with laryngeal histoplasmosis 12 and only 4 cases of patients with an oral lesion. [13][14][15][16] Considering the rarity of histoplasmosis of the head and neck in immunocompetent patients, the paucity of reports, and the constellation of nonspecific symptoms, few clinicians possess the knowledge needed to make a proper diagnosis. In this article, we describe the cases of 2 immunocompetent patients who presented with a head and neck manifestation of histoplasmosis-one involving the oral cavity and the other the larynx.…”
