Disseminated cutaneous granulomatous eruptions associated with myelodysplastic syndrome and acute myeloid leukaemia

Abstract: Two elderly male patients presented with similar, widespread, papular, granulomatous eruptions and developed myelodysplastic syndromes. The first showed histological features of diffuse granuloma annulare and had mild hypergammaglobulinaemia, slightly abnormal liver function and a leucoerythroblastic anaemia. He developed acute myeloid leukaemia (AML) secondary to a background myelodysplastic syndrome and was treated with combination chemotherapy during which his eruption settled; his AML and his skin are in r… Show more

“…GA-associated malignancies have been reported in Hodgkin’s disease [1, 7], non-Hodgkin’s lymphoma [1], chronic lymphocytic lymphoma [1], seminoma [8], myelodysplastic syndrome [9], acute myeloid leukemia [9]and metastatic adenocarcinoma [10]. However, to the best of our knowledge, generalized GA associated with GrMF has not been previously reported.…”

“…[1][2][3][4] In addition, disseminated cutaneous granulomatous eruptions have been described in patients with MDS. [38][39][40] Vestey et al 39 described 2 cases of widespread papular granulomatous eruptions containing epithelioid granulomata with variable numbers of giant cells. The first case resembled granuloma annulare with transformation to acute myeloblastic leukemia and the second case of cutaneous sarcoidosis.…”

“…The skin lesions subsided after his AML was controlled by chemotherapy. 5 Our patient had similar clinical manifestations, with eruptive GA appearing just weeks before AML transformation. However, more observations are needed to clarify the pathogenesis of this phenomenon and to determine whether GA and AML are simply coincidental events or whether the acute onset of generalized eruptive GA can be regarded as a paraneoplastic phenomenon.…”
