“…The chronic indolent course of fungal arthritis can extend for months without signs of systemic disease, often leading to delays in diagnosis and initiation of inappropriate treatment such as intra-articular or systemic steroids. 3 The prednisone course our patient received led to rapid dissemination of the infection, which manifested with worsening purulent knee arthritis that did not grow in bacterial culture, severe acute pulmonary disease, and a skin lesion.…”

“…ere are only a few isolated reported cases of septic arthritis as the presenting symptom of blastomycosis in children [18,19], but this is typically in the setting of concurrent osteomyelitis due to the proposed mechanism of joint involvement being secondary to direct bony spread from an affected limb [7]. One case series described 7 children with disseminated blastomycosis causing osteomyelitis with extension to the joint [20], but none of these children had isolated arthritis.…”

“…Involvement of multiple joints is very atypical. In children, only 3 reports have documented arthritis as a presenting manifestation of blastomycosis 8,9,10 . Of these, only 1 report discusses a child presenting with arthritis of more than 1 joint 10 .…”

“…Our cases suggest that children infected with B. dermatitidis should be evaluated for bone and joint involvement. B. dermatitidis infections of the bone and joint respond well to appropriate antifungal therapy, but can result in poor outcomes if not promptly and adequately treated, including joint destruction and even death 7,10,11,12 . Based on our findings, B. dermatitidis appears to be increasing in incidence in the urban setting and should be considered in the differential of septic arthritis or osteomyelitis for both rural and urban locations.…”
