Dissection of the aorta associated with congenital malformation of the aortic valve

Abstract: The association of congenital aortic valve malformation and aortic dissection is analyzed. Over a 30 year period, 186 patients with non-iatrogenic aortic dissection were studied at necropsy. The aortic valve was tricuspid in 170 (91.4%), bicuspid in 14 (7.5%) and unicuspid in 2 (1.1%). Among the 16 patients with aortic dissection and a congenitally malformed valve, the age at death ranged from 17 to 82 years (mean 52) and 13 (81%) were men. The entrance tear of the aortic dissection was located in the ascendin… Show more

“…It has been reported that patients with bicuspid aortic valve are at increased risk of aneurysms of the ascending aorta, aortic dissections, and rupture. 10 However, in the current study, 2 out of 5 aortic events occurred in tricuspid valve patients, and bicuspid valve disease failed to be a risk factor for aortic enlargement or the occurrence of aortic events. This may be due to the small number of patients.…”

“…Patients with congenitally bicuspid aortic valves have a higher frequency of aortic dissection with tears in the ascending aorta than do patients with tricuspid aortic valves 34 : Ն9% of our patients (5/56) had congenitally bicuspid valves: loss of medial elastic fibers in them was 0 in 1, 1ϩ in 2, and 3ϩ in 2 patients. The bicuspid valves were heavier than the tricuspid valves (mean 1.14 vs 0.62 g).…”

“…[1][2][3] In recent years BAV has also been found to be a risk factor for proximal aortic dilatation and dissection. [4][5][6][7][8][9][10] Recently, a study in a large group of adults suggested that the presence of aortic insufficiency or stenosis may alter the risk of developing ascending aorta dilatation in an adult population with BAV. 11 Indeed, nearly all of the studies conducted to date have reported on adults.…”
