Dissatisfaction with Nasal Tip Shape: Secondary Tip Maneuvers

Abstract: Dissatisfaction with the appearance of the nasal tip is a common compliant in patients seeking revision surgery after rhinoplasty. Revision rhinoplasty is more technically difficult and unpredictable given the frequent presence of scar contracture, impaired skin envelope quality, and missing alar cartilage. This article describes some of the more common causes for tip revision surgery and techniques to address these abnormalities.

“…The lower lateral cartilage is too thick or too wide and the upper lateral cartilage lacks a step transition, resulting in a shallow nasal sulcus. The upper lateral nasal skin is thicker, resulting in a bloated nose and nasal wing 17,18. In addition, because of local early swelling or scar growth after surgery, postoperative hyperplasia at the nasal sulcus can also lead to temporary or long-term disappearance of shallowing, making the nose fatter and more rounded 19–21.…”
