Truncal acne is frequently overlooked in dermatological practice, even though it may result in scars and impact on self-esteem and body image. Therefore, it is important to identify the disease early in order to initiate treatment in time and, thus, to prevent it from worsening and resulting in physical and psychological sequelae. The aim of this review is to provide an overview of what is currently known about truncal acne, its prevalence, aetiology and physiopathology, how its severity is currently evaluated, how to differentiate it from other skin afflictions and current treatment options. A review of literature considering the issue of truncal acne published up to 2019 and available from PubMed was conducted, and in total, 76 articles were selected from PubMed. Currently, only little information about truncal acne is available. Considered as having the same pathophysiology as facial acne, the clinical picture and treatment response seem to differ. Specific acne severity grading systems and quality of life questionnaires as well as a specific treatment algorithm are still lacking. Filling this gap should allow clinicians to assess truncal acne in the best possible way, choosing suitable treatment options, helping patients to improve treatment adherence and quality of life and finally allowing a better management of truncal acne. In conclusion, more knowledge is required to treat more efficiently truncal acne.

Impact of Acne Vulgaris on Psychosocial Status

Background: Most previous research has focused on acne vulgaris characteristics and their association with psychological disorders, such as anxiety and depression. Objective: In the present study, we aimed to show whether acne characteristics are associated with several aspects of psychological well-being namely self-esteem, body satisfaction, anxiety-depression and dermatological life quality. Methods: This was a study of 200 patient with acne and190 healthy controls that was carried out in Antalya, Turkey. Main outcome measures were the Rosenberg’s Self-Esteem Scale, Body Image Concern Inventory (BICI), hospital anxiety and depression scale (HAD) and dermatological life quality index (DLQI). Also we recorded global acne grading score (GAGS) of acne patients. Results: The findings of statistical analysis indicated that patients with acne had lower levels of self-esteem, high depression and HAD score compared with healthy controls (respectively p=0.01, p˂0.01, p=0.01). Also the DLQI scores of severe and very severe group had higher than mild and moderate group (p= 0.013) Conclusion: The emotional well-being of the patients presenting with the acne needs to be recognized more fully, particularly in relation to the low self-esteem, high depression score. The psychosocial impact of acne vulgaris should be valued in the management of patients with this condition. The results of this study raise implications for clinical practice and suggest that a multidisciplinary approach to the management of patient with acne.

Summary Background Many dermatological diseases have a great impact on the psychological state of patients, like urticaria, psoriasis, atopic dermatitis, and acne vulgaris. Finding a “gold standard” biomarker for chronic stress in acne patients is challenging because of the complex etiology of the chronic stress and its variable manifestations. Aims The aim of this study was to evaluate the correlation between serum levels of BDNF and the presence and severity of acne vulgaris and to assess the relationship of this biomarker to both the degree of psychological stress and the quality of patients’ lives (QoL). Patients and Methods Sixty patients with acne vulgaris were included, together with twenty apparently healthy, age‐, and sex‐matched individuals as a control group. Patients filled a Dermatology Life Quality Index (DLQI) questionnaire; both patients and controls filled a Hospital Anxiety and Depression scale (HADS) and perceived stress scale‐10 (PSS) questionnaires. Serum levels of BDNF were measured for patients and controls using ELISA technique. Results Patients with acne had significantly lower levels of BDNF and significantly higher HADS and PSS‐10 questionnaires scores. A significant negative correlation was found between serum levels of BDNF and PSS questionnaire scores. Conclusion Patients with acne are at a high risk to develop chronic stress, anxiety, and depression. BDNF is a good predictor for assessment of chronic stress in such patients.
