A clear link exists between social and economic inequality and poor mental health. There is a social gradient in mental health, and higher levels of income inequality are linked to higher prevalence of mental illness. Despite this, in the late 20th and early 21st century, psychiatric and psychological perspectives have dominated mental health research and policy, obscuring root socioeconomic contributors. Drawing on contemporary research on the social determinants of mental health, with particular reference to Europe and the U.S., this paper argues that a sharper focus on socioeconomic factors is required in research and policy to address inequalities in mental health. Current attempts to move this direction include: evaluation of the impact of economic policies on mental health, community-based partnerships, increased professional awareness and advocacy on socioeconomic factors. This necessitates greater understanding of the barriers to such actions. This paper argues that advancing 'upstream' approaches to population mental health requires an interdisciplinary research vision that supports greater understanding of the role of socioeconomic factors. It also demands collective cross-sectoral action through changes in social and economic policy, as well as economic frameworks that move beyond an exclusive focus on economic growth to embrace collective and societal wellbeing.

Poverty and Family Reunification for Mothers with Substance Use Disorders in Child Welfare

Although prior research has established that maternal substance use disorders (SUDs) are associated with poor foster care outcomes, few earlier studies have sought to differentiate between families with maternal SUDs who reunify and those who do not reunify. This study applied the intersectional social determinants of a health framework to examine the effect of socioeconomic status (SES) risks on the likelihood of reunification for 325 mothers with SUDs and their children in foster care. Multivariate Cox regression models assessed four SES risk factors separately and cumulatively: single parent; income less than $15 000; unemployment; and severe housing issues. The study results reveal that none of the SES factors alone reduced the chances of reunification, but did so in combination. Compared to cases with one risk, cases with three risks were 39.1 per cent less likely to reunify and four‐risk cases were 49.3 per cent less likely to reunify. Cases with zero, one and two risks did not significantly differ. The primary implication of this study is that families with maternal SUDs in child welfare are a heterogeneous group whereby reunification depends on SES risk severity. In practice, these findings suggest that improving reunification for families with maternal SUDs may be aided by reducing the total number of socioeconomic risk factors, rather than eliminating specific risks. © 2018 John Wiley & Sons, Ltd. Key Practitioner Messages Families affected by parental SUDs in child welfare are a heterogeneous group whose outcomes vary based on the severity of socioeconomic risk. Mothers with SUDs in child welfare are significantly worse off socioeconomically compared to fathers with SUDs. The impact of SES on reunification outcomes depends on the accumulation of risk, rather than individual factors. Substance abuse treatment must be augmented with efforts to reduce the cumulative SES burden on families.

Background For youth probationers, it is important to understand how mental health and substance use predict their suicidal thoughts and behaviors (STB) to identify interventions to reduce their psychological distress. Objective In this study, risk and protective factor indicators based on the Youth Assessment and Screening Instrument (YASI) Full Assessment were used to explore STB of youth probationers. The study's overarching aim was to examine the associations of psychological distress and other risk and protective factors with youth probationers' STB based on a Social Determinants of Health framework. Method This cross-sectional secondary analysis reviewed YASI records from a sample of 11,607 probationers of age 12-18 years within a large urban setting. Results The study used logistic regression models to assess risk and protective factors for STB odds (5.79%, n = 672 positive endorsement). African Americans were less likely to report STB, and girls were much more likely than boys to report STB across risk factors. Mental health disorders and substance use increased STB risk. Conclusion The findings underscore the need for screening and treatment of psychological distress for youth probationers. This study discussed these findings, strengths and limitations, and directions for future research.
