Scalp Dermatoscopic Findings in Androgenetic Alopecia and Their Relations with Disease Severity

BackgroundClinicians are searching for new methods to diagnose and predict the course of androgenetic alopecia noninvasively.ObjectiveOur aim is to evaluate trichoscopic findings and their relations with disease severity in androgenetic alopecia.MethodsThe videodermatoscopic findings of 143 female and 63 male patients with androgenetic alopecia were compared with each other, with those of healthy subjects (n=100), and with those of patients with other nonscarring alopecias (n=208). Mann-Whitney U-test, χ2 analyses, and logistic regression analysis were used for statistical analysis.ResultsNo statistically significant relation was found between trichoscopic findings and severity in male androgenetic alopecia (MAGA) on the basis of the modified Hamilton Norwood scale (among 7 degrees); however, multihair follicular unit and perifollicular pigmentation were related to low severity whereas white dots, honeycomb pattern pigmentation, and brown dots were related to high severity. On the other hand, according to the Ludwig classification, arborizing red lines were related to low severity and brown dots were related to high severity, whereas there was no difference in stages between the Ebling and Olsen classifications in female androgenetic alopecia (FAGA). In the characteristic trichoscopic findings in this study, perifollicular pigmentation was found as a normal feature of the scalp, whereas multihair follicular unit and honeycomb pigment pattern, which were previously considered as normal features, were observed to be related to androgenetic alopecia.ConclusionNo relation was found between MAGA severity and trichoscopic findings, as well as between FAGA severity according to different disease severity classifications and trichoscopic findings.

This study was conducted at the experimental area of the Field Crops Department of Çukurova University (37°01′N, 35°18′E) in the 2010 and 2011 growing seasons to determine the effects of sole or combined applications of 3 organic materials (poultry litter, cattle manure, and leonardite) with supplemental inorganic fertilizer on the forage yield and quality of silage maize. The field trial was arranged in a complete randomized block design with 3 replications. Poultry litter (PL) and cattle manure (CM) were applied based on P or N requirements (PL-P, PL-N, CM-P, and CM-N) of the crop, whereas leonardite was applied in only 1 dose (500 kg ha -1 ) and was also combined with 3 inorganic fertilizer doses (100%, 75%, and 50% of the recommended inorganic fertilizer dose, called LEO-100, LEO-75, and LEO-50, respectively). The results show that the highest values of dry matter (DM) and crude protein (CP) yields (18.3 t ha -1 and 1652 kg ha -1 , respectively) were observed in LEO-100, whereas the lowest (11.2 t ha -1 and 758 kg ha -1 , respectively) were observed in PL-N. In terms of forage quality parameters (CP, acid detergent fiber, neutral detergent fiber, and relative feed value), the P-based poultry litter and cattle manure treatments (PL-P and CM-P) gave similar results to conventional fertilization treatment, whereas the N-based treatments (PL-N and CM-N) significantly decreased forage quality. The combined use of organic materials with inorganic fertilizers instead of a conventional fertilization regime may be effective in maintaining forage supply but N-based treatments have the risk of restricting forage production from silage maize.

ÖzetAmaç: Bu çalışmada kliniğimize başvuran telogen effluviumlu kadınlarda "Dijital Fototrikogram" (Trichoscan) ile "Darı Ekstresi, Buğday Tohumu Yağı, L-Sistin, Kalsiyum Pantotenat" formulasyonunun etkisinin değerlendirilmesi amaçlandı. Gereç ve Yöntem: Ocak 2008-Kasım 2008 tarihleri arasında, Gazi Üniversitesi Tıp Fakültesi Dermatoloji Polikliniği'ne diffüz saç dökülmesi şikayetiyle başvuran kadın hastalar değerlendirildi. Hastalara dijital fototrikogram testi frontal ve oksipital bölgelerde uygulanarak anagen ve telogen oranları saptandı. Klinik görünüm ve dijital fototrikogram sonucu telogen effluvium tipi diffüz alopesi ile uyumlu, 18-50 yaşları arasında 53 kadın hasta çalışma kapsamına alındı. Hastalara darı ekstresi 140 mg, buğday tohumu yağı 271 mg, L-sistin 2 mg, kalsiyum pantotenat 10 mg'dan oluşan kapsül günde 3x1 dozunda, 3 ay süre ile verildi. Çalışma kapsamına alınan hastaların tedaviye yanıtlarının değerlendirilmesinde günlük dökülen saç sayısı, saç çekme testi ve dijital fototrikogram kriter olarak alındı. Bulgular: İlk değerlendirmede günlük dökülen yaklaşık saç sayısı ortalaması 84,1±55,9 iken verilen 3 aylık tedaviden sonra günlük dökülen saç sayısı ortalaması 44,9±28 olarak saptandı. Hastaların tedavi sonrasında günlük dökülen ortalama saç sayısındaki azalma istatistiksel olarak anlamlı bulundu. Hastalarımızın tedavi öncesi saç çekme testi ile tedavi sonrası saç çekme testi arasında istatistiksel olarak anlamlı farklılık saptandı. Tedavi öncesinde ortalama anagen değeri frontal bölgede 69,0±11,2, oksipital bölgede 72,3±9,4 olarak saptandı. Tedavi sonrasında frontal bölge anagen oranı 76,4±11,2'e, oksipital bölge anagen oranı 79,9±8,6'a yükseldi. Tedavi sonrasında frontal ve oksipital bölgede anagen oranındaki artma istatistiksel olarak anlamlı olarak kabul edildi. Sonuç: Çalışmamızda "L-sistin, kalsiyum pantotenat, buğday tohumu yağı ve darı ekstresinden oluşan formulasyon"unun telogen effluvium tedavisinde etkili olduğu düşünülebilir. Ancak daha geniş alopesili hasta gruplarında plasebo kontrollü çalışmalar gereklidir. Sum maryBackground and Design: In this study, we aimed to evaluate the effects of a formula composed of millet extract, wheat germ oil, calcium pantothenate and L-cystine in women with diffuse alopecia by using Digital Phototrichogram (Trichoscan). Materials and methods:We evaluated female patients who presented to Gazi University Faculty of Medicine Department of Dermatology between January 2008 and November 2008 with the complaint of diffuse hair loss. Digital phototrichogram was performed in the frontal and occipital regions of the patients. Using this method, anagen:telogen ratios were determined. 53 female patients whose ages were between 18 and 50 and who were diagnosed as having telogen effluvium type diffuse alopecia were included in the study. The patients were given a capsule containing 140 mg millet extract, 271 mg wheat germ oil, 2 mg L-cystine and 10 mg calcium pantothenate three times a day for three months.
