Disease-specific differences of left ventricular rotational mechanics between cardiac amyloidosis and hypertrophic cardiomyopathy

Abstract: Left ventricular (LV) twist (LVT) and untwisting (LVUT) rate are global and thorough parameters of LV function. The aim of the present study was to investigate the differences in LV rotational mechanics between patients with cardiac amyloidosis (CA) and hypertrophic cardiomyopathy (HCM). Twenty consecutive patients with CA, 20 consecutive patients with HCM, and 20 consecutive subjects without evidence of structural heart disease were included. Cardiac magnetic resonance (CMR) with late gadolinium enhancement (… Show more

“…37 In hypertrophic cardiomyopathy, the relative increase in wall thickness produces larger radial differences between the endocardium and the epicardium resulting in augmentation of peak LVT. 38 The writhing area (ie, area of null rotation) 3 is displaced apically in hypertrophic cardiomyopathy because of myofiber disarray. 18 Interestingly, LVT and untwist are found to increase in genotypically positive hypertrophic cardiomyopathy carriers, despite having normal LV wall thickness.…”

“…20 LV rotational indices of twist and untwist rate have been used to differentiate cardiomyopathies and have revealed disease-specific patterns of rotation in hypertrophic cardiomyopathy and cardiac amyloidosis. 36 There is mounting evidence from tissue Doppler and STE that mechanical dyssynchrony is associated with adverse outcomes. 37,38 Recently, the feasibility of CMR-FT for the assessment of dyssynchrony has been demonstrated.…”

“…Besides quantifying biventricular function parameters and LV wall thickness with high accuracy and reproducibility, cMR is able to yield information about replacement fibrosis, through the use of late gadolinium enhancement (LGE) imaging, and interstitial fibrosis, through the use of T1 relaxation measurement techniques [14]. In addition, assessment of LV myocardial mechanics directly from cine cMR images is possible through the use of a recently introduced featuretracking cMR software system [15,16].…”
