Discussions Regarding Reproductive Health for Young Women With Breast Cancer Undergoing Chemotherapy

Abstract: Many women fail to recall discussions regarding the reproductive health impact of chemotherapy. Demographic, psychological, and disease-related variables are related to recalling such discussions. Counseling about premature menopause and fertility changes is an overlooked aspect of preparation for adjuvant chemotherapy in young premenopausal women with breast cancer. Future research should explore this issue further.

“…Previous surveys of breast cancer survivors have demonstrated that a large proportion of patients do not recall their doctors having discussed with them the risks of early menopause and infertility prior to the onset of adjuvant cancer treatment [26]. Such retrospective surveys are subject to recall bias, especially as discussions take place at a time when patients may not be able to process the information they receive [27].…”

“…It has been reported in a US study, that 45 % of oncologists did not routinely discuss fertility preservation treatment with women of reproductive age [15] despite infertility being rated as one of the most important issues amongst women with cancer [11]. There are various factors contributing to this referral barrier.…”

“…Fertility preservation is an important part of the multidisciplinary management of cancer in reproductive-aged women, given the known gonadotoxity of various chemo/radiotherapy regimens and patients' concern for their future fertility [11]. Although the incidence of cancer increases with age, a significant proportion of cancer diagnoses are made in young women of reproductive age.…”

“…Canadian fertility specialists have perceived both inadequate education and inadequate awareness of infertility risk, together with a lack of support from oncology providers, as the top reasons for poor referral volumes 3 . Among 1093 postpubertal patients, male and female, the rates of physician-led fertility preservation discussions varied from as low as 34% to 86% [26][27][28][29] . Furthermore, patients rely on endorsement from their oncology health care providers in deciding whether to pursue fertility preservation 25,27,30 , even with multiple knowledge resources in place 31 .…”
