Discrepancies Between Glycosuria and Home Estimates of Blood Glucose in Insulin-treated Diabetes Mellitus

Abstract: Glycosuria can be a misleading indicator of blood or plasma glucose levels. Thus glycosuria may be present when blood glucose levels are within the normal fasting or postprandial range, and it may be absent when the blood glucose is distinctly above normal. In such patients the blood glucose must be measured, preferably by the patient, as a guide to insulin and other therapy. However, urine glucose tests are valid indicators in a minority of patients and are essential in all patients for the detection of aceto… Show more

“…Furthermore, because we have used information obtained in normal life, our results are more relevant to the everyday experience of the patients than those achieved on a metabolic ward. We have failed to detect any significant variation of threshold over several months, confirming the original observation of Lawrence 1 and in contrast to the recent work of Ohlsen et al 9 When we applied this type of analysis to results obtained by pregnant women, we have been able to detect changes in threshold during the pregnancy. The technique does therefore seem to be sensitive to such variations when they exist.…”

“…24 We did not consider a record of self-determined fractional glycosuria from our patients, because it would be neither reliable nor objective. 25 The average FBS levels in our study were similar to those of other reports of diabetic outpatient clinics, partly reflecting the difficulties in obtaining metabolic normalization with 1526 However, it is to be noted that an U N M E A S diet has been successfully applied in the treatment of type I diabetes designed for tight control with intense insulin therapy. 27 T h e high prevalent mean fasting glycemia and the relatively constant insulin dosages also suggest that more vigorous insulin treatment was needed in this outpatient diabetic population.…”

“…In addition, scrupulously performed urinary glucose determinations have been demonstrated to be inaccurate as frequently as 60% of the time. 13 ' 14 Hence, glycosuria may not always correlate with blood glucose concentrations or the degree of diabetic control. In contrast, blood glucose monitoring has been shown to give accurate and reliable data that the diabetic patient can use to guide his self-management regimen.…”
