Discordance between Perceived and Actual Cancer Stage among Cancer Patients in Korea: A Nationwide Survey

Abstract: PurposeWe assessed the accuracy of communication between doctors and patients by evaluating the consistency between patient perception of cancer stage and the medical records, and analyzed the most influential factors of incongruence among cancer patients at 10 cancer centers across Korea.MethodsInformation was gathered from cancer patients at the National Cancer Center and nine regional cancer centers located in every province of Korea between 1 July 2008 and 31 August 2008. Data were analyzed using Pearson's… Show more

“…This has been the study limitation. A study in Korea implicitly showed that there were knowledge gaps between patients' perceived and actual stage of cancer (17). The lower stage found in cervical cancer respondents in this study possibly happened due to the same reason with the above study.…”

“…The waiting time to get health service in PHC is also being a key performance indicator of PHC work performance. One way to test the accuracy of communication between doctors and patients is by evaluating the consistency between patient perception of cancer stage and the stage given in medical records (17). Other than communication reason, there is a possibility that cancer stage was not identified yet to limitation in laboratory and radiology tests related to cancer diagnosis, such as waiting time for biopsy because all respondents used public health services.…”

“…This finding might be explained to some extent by Korean culture, in which half of patients with advanced cancer are not accurately informed about the stage of their cancer (Lee et al, ). Due to the cultural belief that talking about death is potentially harmful to the patient, family and health‐care providers frequently do not communicate openly with patients about the end of life (Shim et al, ). When family surrogates do not know patients' wishes, they will experience great difficulty in choosing the best treatment for patients at their own discretion.…”

“…To identify predictors of discordant view of the patients and caregivers, we performed multivariate regression analyses with mixed effect model ( xtmelogit command by STATA software). The potential patient‐level predictors include patient's age at diagnosis, sex, education, cancer stage, decision control preference (chosen because of potential correlation with information need), and depression (chosen because of potential association with preference for prognosis information or general illness perception). Caregiver characteristics included age, sex, and education.…”
