Disability and Medicare Costs of Elderly Persons

Abstract: Widely studied as a predictor of long‐term care use, functional disability is also an important explanatory variable for acute care costs spent for the elderly. As policy makers contemplate ways to control rapidly growing Medicare expenditures, such as increasing the enrollment of beneficiaries into managed care programs, knowledge about the relation between functional disability and Medicare costs is likely to become more important for the design of program features. The research described here uses data from… Show more

“…However, the impact on Medicare spending for these two groups was far from equal. The additional cost associated with transitioning to more severe disability significantly outweighed any savings from transitioning to less severe disability, a result consistent with prior research (Hoffman et al, 2010;Liu et al, 1997). The one exception to this relationship was recovering from severe disability, which generated significant savings in excess of the costs of transitioning into a severe disability state.…”

“…Women report fewer physician visits and are less likely to receive hospital care than men (Wolinsky & Johnson, 1991). In addition, health care utilization increases with age (Liu, Wall, & Wissoker, 1997), and some use of health services differ by marital status (Crystal, Johnson, Harman, Sambamoorthi, & Kumar, 2000;Mutran & Ferraro, 1988). Therefore, potential gender and ethnicity/race utilization differences, controlling for other predisposing factors, are investigated.…”

“…This is an important question from a policy perspective, because it addresses inequities in health care utilization among people with similar burdens of health problems. Health needs, including functional limitations and chronic conditions, increase health care utilization (Liu et al, 1997). At the same time, health needs differ by gender and ethnicity.…”
