Direct Measurement of Multiple Vena Contracta Areas for Assessing the Severity of Mitral Regurgitation Using 3D TEE

Abstract: Direct measurement of multiple VC areas using 3D transesophageal echocardiography allows for assessing MR severity in patients with multiple jets, particularly for MR degrees greater than mild and in cases of more than 2 jets, for which geometric assumptions may be challenging.

“…This technique may have a limited ability to obtain reliable measures with multiple regurgitant jets; in the case of multiple MR jets, the fact that the respective widths of the VC are not additive must be taken into account, and in these cases the use VCW method is not recommended 21, 31, 42…”

“…7.38 Diff erences in the asymmetry of the vena contracta area as visualized by RT3DE in patients with degenerative mitral regurgitation (MR), mitral valve prolapse (MVP), and functional MR. Source [14]; Copyright American Society of Echocardiography vided further validation of RT3DE assessment of the asymmetric VCA by comparison against independent methods [82] and the proof of superiority of 3D VCA measurements compared to 2D VCA measurement in both central and eccentric jets [83] as well as in multiple jets [84]. The overwhelming variability of shape, size, and number of VCAs in a spectrum of patients with both functional and organic MR is shown in .…”

“…Methods of assessment include quantitative Doppler and direct planimetry of effective regurgitant orifice area by 3D reconstruction. 27,28 However, after transcatheter devices are placed, these methods become less accurate or invalid because of Doppler flow acceleration and acoustic artifacts from devices. In the posttranscatheter edge-to-edge repair setting, multiple mitral regurgitation jets may be difficult to assess on TEE color Doppler imaging because of artifacts from the device(s).…”
