Direct Comparison of Single-Scan Autoradiographic with Multiple-Scan Least-Squares Fitting Approaches to PET CMRO2 Estimation

Abstract: The time course of local cerebral radioactivity concentration after bolus inhalation of oxygen gas labeled with O-15 was measured in a rapid dynamic sequence of positron tomographic images. Four normal subjects were studied at rest. In each study, 15 multiple-slice image sets were acquired over a 3-min period in a Scanditronix model 384 tomograph. The radioactivity concentration in arterial blood was measured at 1-s intervals by means of a pump-fed flow-through detector. Pump effluent was directed to discrete … Show more

“…Considering the above observations, the use of a separately measured value of H2150 clearance (or CBF) may introduce an error into the calculation of oxygen consumption by the three-step method. In addition, the separately measured clearance may not remain representative during the 30 to 40 minutes required to measure oxygen consumption (Meyer et al, 1987;Correia et al, 1985;Holden et al, 1988;Lam mertsma et al, 1986).…”

“…Meyer et al (1987) ob tained reliable estimates of oxygen consumption from a single oxygen bolus inhalation study but, on the basis of the current model, showed that simul taneous estimates of blood flow and blood volume were unreliable. Holden et al (1988) stressed that accurate knowledge of the partition volume of wa ter is essential for the estimation of oxygen con sumption from a single oxygen study with the cur rent model. Lammertsma et al (1986) The new model solution overcomes some of the shortcomings mentioned above and yields oxygen consumption in a single dynamic [,50]OZ inhalation study of only 3-min duration.…”

“…There have been other attempts to obtain CBF and CMRO 2 images more rapidly (Huang et al, 1986;Holden et al, 1988;Meyer et al, 1987;Ohta et al, 1992;Ho and Feng, 1999). Mathematical refinement has allowed images to be generated from data from a single scan alone on a bolus inhalation of 15 O 2 .…”

“…The availability of a bolus inhalation protocol would enable more widespread use of 15 O 2 gas measurements. Such a protocol has previously been described for cerebral oxygen consumption (3,4), but its use for myocardial imaging has not been described and presents several additional challenges, mainly because of increased scatter and image noise and the presence of large amounts of radioactivity in the lungs during the bolus inhalation period. The measured radioactivity concentration in the myocardial wall is affected both by the limited spatial resolution of PET relative to the thickness of the myocardium and by spillover from the left-and right-ventricular cavities and the lungs.…”
