Diphenylhydantoin‐induced Gingival Hyperplasia: its Relationship to Dose and Serum Level

Abstract: SUMMARY The incidence and severity of gingival hyperplasia was studied in 227 patients treated with diphenylhydantoin (DPH). 67 per cent of the patients had gingival changes, the severity of which was proportional to both the dose and the serum level of the drug. In patients taking DPH in recommended doses of 3 to 8mg/kg/day, the incidence of hyperplasia was 78.2 per cent, and in those with serum levels of DPH within the therapeutic range of 10 to 20mg/litre the incidence was 93.2 per cent. “…The incidence of phenytoin-induced gingival enlargement as reported by a study by Kimball was found to be 57% while other studies in relation to incidence of phenytoin-induced gingival enlargement have revealed wide incidence range of 20-40%[930] in some studies to 6-79% in others[41314212630] while 3-93% in few other studies[1819] and 50% in institutionalized epileptic patients (Seymour, 1993) as reported in the literature. The incidence of gingival overgrowth in the normal population has been reported to be between 4% and 7.5%.…”

“…[3132] Chronic administration of phenytoin has been associated with a number of adverse effects. [333435] Phenytoin-induced gingival overgrowth is one such most frequently reported gingival lesion which was first described in 1939. [2021]…”
