Purpose To study the prevalence, the clinical features, and the visual prognosis without treatment of polypoidal choroidal vasculopathy (PCV) in a large series of Greek patients presenting with exudative maculopathy. Methods The medical records, photographs, as well as fluorescein and indocyanine green (ICG) angiograms of a series of 268 consecutive elderly white Greek patients, who were originally diagnosed as having exudative age-related macular degeneration (AMD) were reviewed retrospectively. Results In all, 22 of the 268 (8.2%) patients initially suspected of having AMD were ultimately diagnosed with PCV. In 15 of the 22 (68.2%) patients with PCV, the polypoidal lesions were located in the peripapillary area. Large soft drusen were present in only two fellow eyes of the 10 (20%) patients with unilateral PCV compared with 120 fellow eyes of the 148 (81.1%) patients with unilateral AMD. At the last examination, 11 of the 22 (50%) patients with PCV and 120 of the 246 (48.8%) patients with AMD presented a visual acuity of less than 6/60 in at least one eye due to scar formation in the macula. Conclusions PCV is not an infrequent disease in Greece. A measurable number of Greek patients with findings suggestive of exudative AMD will instead have PCV. ICG angiography is important in differentiating between these two clinical entities. In Greeks, polypoidal lesions are predominately peripapillary and are not usually associated with macular drusen in the fellow eye. PCV and exudative AMD do not differ significantly in terms of their natural course and visual prognosis in Greek patients.

Background To describe the efficacy of photodynamic therapy (PDT) with verteporfin for the treatment of polypoidal choroidal vasculopathy (PCV). Methods This is a retrospective interventional case series of 41 eyes of 40 patients with angiographic evidence of PCV, which had PDT with verteporfin. Pre-treatment best-corrected visual acuity (BCVA) was measured and patients were followed up for at least 12 months with BCVA recorded at each visit. Results The mean follow-up time was 23.7 months. Seven of 10 eyes (70%) with juxtafoveal lesions and 17 of 31 eyes (54.8%) with subfoveal lesions had stable or improved vision (loss of p3 lines) at the last follow-up. The mean number of treatments was 1.90. Thirty-three eyes (80.5%) had dry, quiescent scars at last follow-up, six eyes (14.6%) had persistent leakage, and two eyes (4.9%) had evidence of choroidal neovascularisation. Conclusion Our results indicate that 24 of 41 eyes (58.5%) with serosanguinous maculopathy secondary to PCV treated with PDT had stable or improved vision (loss of p3 lines) after a mean follow-up of almost 2 years. However, in view of the retrospective nature of this study, the true efficacy of PDT for PCV would have to be evaluated with a larger randomised controlled trial.

To determine the nature and frequency of polypoidal choroidal vasculopathy (PCV) in a series of patients suspected of having neovascularized agerelated macular degeneration (AMD).Methods: A prospective analysis of 167 consecutive, newly diagnosed patients aged 55 years or older with presumed neovascularized AMD was performed. All patients were examined with fundus biomicroscopy as well as fluorescein and indocyanine green angiography.Results: Choroidal neovascularization secondary to AMD was diagnosed in 154 (92.2%) of 167 patients; 13 (7.8%) patients had PCV. The patients affected by PCV were younger than those with AMD (P = .01). Peripapillary choroidal neovascularization was seen in 3 (1.9%) of 154 patients with AMD and 3 (23.1%) of 13 patients with PCV (P = .006). Significant drusen were present in 63 (70%) of 90 fellow eyes with unilateral AMD compared with only 1 (16.7%) of 6 eyes with PCV (P = .02). Only 5 patients with AMD (3.2%) were nonwhite compared with 3 patients with PCV (23.1%) (P = .02).Conclusions: A measurable number of elderly patients with findings suggestive of neovascularized AMD and serosanguineous macular manifestations will instead have PCV. Polypoidal choroidal vasculopathy can occur in any sex or race, but is more commonly seen in the peripapillary area, without associated drusen, and in nonwhite patients. It is important to differentiate AMD from PCV because there are significant differences in the demographic risk profile, natural course, visual prognosis, and management of these patients.
