Dimethyl fumarate–induced lymphopenia in MS due to differential T-cell subset apoptosis

Abstract: Objective:To examine the mechanism underlying the preferential CD8+ vs CD4+ T-cell lymphopenia induced by dimethyl fumarate (DMF) treatment of MS.Methods:Total lymphocyte counts and comprehensive T-cell subset analyses were performed in high-quality samples obtained from patients with MS prior to and serially following DMF treatment initiation. Random coefficient mixed-effects analysis was used to model the trajectory of T-cell subset losses in vivo. Survival and apoptosis of distinct T-cell subsets were asses… Show more

“…19,60,62,[75][76][77][78] The clinical observation of the preferential reduction of peripheral CD8 + compared with CD4 + T cells was also confirmed in vitro. Ghadiri et al 62 found that DMFinduced apoptotic cell death was approximately twice as marked for CD8 + T cells compared with CD4 + T cells. However, the exact mechanism of this effect in treated psoriasis 24,26,27 and MS patients 58,60-65 remains unknown.…”

“…For all T-cell subsets examined, most in vivo changes occurred within the first 6 months of FAE 27 or DMF treatment. [60][61][62]65 Primarily based on the recently published case series of 19 patients with FAErelated PML, 29,30 we recommend a safety threshold of 200/lL for a CD4 + T-cell count 26 and 140/lL for a CD8 + T-cell count. This increased vigilance in T-cell monitoring appears to be particularly warranted in older patients, 49 especially those who are older than 40 years of age.…”
