Fundamentos: La cesárea electiva es un procedimiento quirúrgico que ha incrementado en todo el mundo en los últimos años. Las morbilidades respiratorias constituyen las complicaciones más frecuentes en neonatos nacidos por esta vía, en especial por la ausencia de trabajo de parto. Objetivo: Determinar, a partir de una revisión bibliográfica sistemática, la cesárea electiva como factor de riesgo para la morbi-mortalidad por distress respiratorio en recién nacidos. Métodos: se incluyeron 14 artículos en relación a este tema los cuales cumplieron determinados criterios de inclusión. La búsqueda se realizó en diferentes revistas como Scielo, Cochrane, Medwave, Redalyc, PubMed, entre otros, publicadas en los últimos 5 años, en idioma inglés y español. Se empleó el software Mendeley para la organización de la información. Resultados: los países subdesarrollados presentan tasas de cesáreas electivas superiores al 40%, en especial en el sector privado. La morbi-mortalidad respiratoria neonatal aumenta por cesárea electiva siendo hasta 3 veces superior a comparación del parto vaginal. La Taquipnea Transitoria del Recién Nacido fue la entidad más frecuente del Síndrome de Distress Respiratorio Neonatal. Conclusiones: la cesárea electiva constituye una problemática en países subdesarrollados ya que trae consigo complicaciones fetales importantes como la Taquipnea Transitoria del Recién Nacido, patología que se puede prevenir: en primera instancia educando adecuadamente a la madre, la adopción de medidas como el trabajo de parto previo en pacientes con antecedentes de cesárea anterior, el uso de corticoides antenatales y la administración de oxitocina.

BackgroundThe aim of this study was to evaluate the behavioral uptake and ability to diagnose pituitary adenoma (PA) using tumor-seeking radiopharmaceuticals, and to provide a semiquantitative analysis of tracer uptake in the pituitary region.Patients and methodsThe study included 33 (13 hormonally active and 20 nonfunctioning) patients with PA and 45 control participants without pituitary involvement. All patients (n=78) underwent single photon emission computed tomography (SPECT) imaging with technetium-99m-labeled hydrazinonicotinyl-tyr3-octreotide (99mTc-HYNIC-TOC), dimercaptosuccinic acid (99mTc(V)-DMSA) and hexakis-2-methoxyisobutylisonitrile (99mTc-MIBI). A semiquantitative analysis of abnormal uptake was carried out by drawing identical regions of interest over the pituitary area and the normal brain on one transverse section that shows the lesion most clearly. The pituitary uptake to normal brain uptake (P/B) ratio was calculated in all cases.ResultsThe result of this study confirms that the SPECT semiquantitative method, with all three tracers, showed statistically significant differences between the PA group and the controls. However, 99mTc-HYNIC-TOC scintigraphy could have the highest diagnostic yield because of the smallest overlap between the P/B ratios between adenoma versus nonadenoma participants (the receiver operating characteristic curve P/B ratio cut-off value was 13.08). In addition, only 99mTc-MIBI SPECT have the diagnostic potential to detect secreting PAs, with statistically significant differences between groups (P<0.001), with an receiver operating characteristic curve P/B ratio cut-off value of 16.72.ConclusionA semiquantitative analysis of increased focal tracer uptake in the sellar area showed that 99mTc-HYNIC-TOC is a highly sensitive and reliable tumor-seeking agent for detecting PA, whereas 99mTc-MIBI SPECT is a highly sensitive and specific method in differentiating hormone-secreting pituitary tumor.

Objective: In dynamic renal scans, 99m Tc-diethylenetriamine pentaacetic acid (DTPA) is copiously used and it yields information about the renal blood-flow and the excretory capacity. 99m Tc-dimercaptosuccinic acid (DMSA) is used for static renal imaging and can likewise uncover the renal cortical structure. This work was intended to evaluate whether DMSA can be declared as a radiopharmaceutical in dynamic renal scans or not. It also aimed at comparing the outcomes procured from DMSA and with DTPA examinations. Materials and Methods: A comparison of the information gained from the renograms of 47 subjects (normal subjects and subjects with abnormal renal function having: obstructive nephropathy, bilateral nephropathy, hydronephrosis, reduced or non-function kidney, and/or atrophic kidney) who had 99m Tc-DMSA and 99m Tc-DTPA dynamic scintigraphies utilizing the same protocol. Results: A strong positive correlation had come in view on the evaluation of the left kidney, right kidney, and total glomerular filtration rate (GFR) from 99m Tc-DTPA and 99m Tc-DMSA renograms. In addition, the estimation of the time of the peak height from 99m Tc-DTPA and 99m Tc-DMSA renograms for the right and left kidneys demonstrated as non-significant varieties and a strong positive correlation for both the normal subjects and subjects with kidney abnormalities. Conclusion: In the light of the outcomes gained from the present investigation, it could be prescribed that 99m Tc-DMSA can favorably replace 99m Tc-DTPA in dynamic renograms in the case of the renal conditions that had been examined in the present work. As scheduled, this will be cost-effective and time saving and will decrease the radiation dose administered to the subjects. Also, the same qualities of results are accomplished with both radiopharmaceuticals in normal subjects and subjects with abnormal renal function.
