Digitalis Withdrawal in Elderly Nursing Home Patients

Abstract: Nursing home patients were studied to determine the usefulness of a maintenance dose of digoxin in elderly patients with normal sinus rhythm. Of 64 patients, 26 were identified to be on digoxin. Thorough history and physical examination were done on all the patients. Baseline electrocardiogram showed normal sinus rhythm in 19 patients, who were observed very closely for the period of four months after withdrawal of digoxin. Eighteen of 19 patients did well without digoxin, which suggests that most of the elder… Show more

“…The median time to ADWE occurrence after drug withdrawal was 35 days (Figure). Twenty-six ADWEs (36%) were classified as major since they were associated with increased health services utilization; 19 resulted in hospitalization, 6 in emergency department or urgent care clinic evaluation without hospitalization, and 1 in prolonged hospital¬ ization.…”

“…A patient may originally receive digoxin for an episode of heart failure induced by specific precipitants, but the digoxin is continued long past the therapeutic need. The literature has suggested for years that digoxin use in elderly patients is often unnecessary 21 , 22 and may be withdrawn without detriment 23–26 . Despite this evidence, long‐term digoxin use continues; digoxin is one of the most frequently prescribed drugs in elderly nursing home patients 27 .…”
