Venous thromboembolic events (VTE) in children are mostly related to central venous lines (CVL), and are located in the central upper venous system. The incidence of VTE in children with CVL is significant. However, the majority of CVL-related VTE do not present with typical symptoms or are not recognized due to underlying disease. Asymptomatic VTE still cause significant venous obstruction and are associated with short-term and long-term clinical complications. Because the clinical diagnosis of CVL-related VTE is unreliable, screening by objective radiographic testing is required. In the upper venous system, ultrasound is insensitive for the VTE in the central venous system and venography is not sensitive for jugular VTE. Therefore, a combination of ultrasound and venography is required for accurate diagnosis of CVL-related VTE in the upper venous system. Whether ultrasound alone is accurate for CVL-related VTE in the lower venous system is uncertain. Magnetic resonance venography will likely prove a valid alternative for diagnosis of VTE both in the upper and lower central venous system, and may be combined with magnetic resonance pulmonary angiography to screen for pulmonary embolism.

44 ResumoOs autores fazem uma revisão bibliográfica de trombose venosa renal (TVR), causa freqüente de insuficiência renal aguda no período neonatal. Seu quadro clínico evolutivo é variável e, embora haja concordância quanto ao tratamento da doença de base e às medidas gerais, a eficácia da terapêutica fibrinolítica e anticoagulante permanece indefinida. Os 5 casos de TVR descritos foram atendidos no nosso serviço nos últimos 5 anos. Quatro dos 5 foram acompanhados, um necessitou nefrectomia para controle de hipertensão arterial. O objetivo deste estudo é descrevê-los comparando-os com os dados da literatura, além de discutir as condutas terapêuticas atuais. Clinical manifestations and prognosis are unsteady and, although there is agreement about the management of underlying conditions and general abnormalities, the relative role of anticoagulant and fibrinolytic therapy has not been clearly established. The 5 RVT described cases were treated in our hospital in the last 5 years. Four out of five were followed and one of these was submitted to nefrectomy to control arterial hypertension. Kleis 2 Introdução A Trombose Venosa Renal (TVR), obstrução parcial ou total do sistema venoso que drena o rim, foi descrita pela primeira vez por Rayer em 1840 1 . É causa freqüente de insuficiência renal aguda no período neonatal, em que a instalação do processo é favorecida pelo padrão hemodinâmico, circulação lenta, tendência à hiperviscosidade, hematócrito elevado e vasos com calibre reduzido 2 .Pode ocorrer em qualquer idade, mas cerca de 75% dos casos ocorrem no PNN e apenas 7% em crianças maiores de 1 ano 3 .Sua exata incidência é indeterminada, mas o relato de casos tem aumentado e acredita-se não ser tão rara quanto se supunha 2 .No período neonatal ocorre predomínio no sexo masculino (1,9:1,0), mas esse predomínio diminui nos meses subseqüentes e, após o primeiro ano a incidência é de 1:1.Em nosso serviço, nos últimos 5 anos, tivemos 5 casos de TVR. O objetivo deste estudo é fazer uma revisão bibliográfica sobre o tema, descrever os casos e sua evolução, e compará-los com os dados da literatura, além de apresentar e discutir as condutas terapêuticas atuais.
