Digital-Droplet PCR for Quantification of CD19-Directed CAR T-Cells

Abstract: CD19-directed CART cells (CD19-CAR) have demonstrated remarkable clinical results in patients suffering from refractory or relapsed lymphoma and acute lymphoblastic leukemia. In order to further optimize follow-up, to explain treatment failure, and to control adverse events biomarkers for monitoring of response are urgently needed. Peak expansion and persistence are correlated with response rates and severity of side effects. However, no standardized method or commercially assay for CD19-CAR measurement is est… Show more

“…Whole-body PET-CT scan 1 and 3 months later (days +30 and +90) showed a complete response (CR) with only residual lymph nodes without metabolic activation ( Figures 3C, D ) . The side effects and clinical response were in agreement with the expansion of anti-CD19 CAR T-cells in quantifications by ddPCR, as has recently been reported by our group ( Figure 4 ) ( 13 ).…”

“…In brief, genomic DNA was extracted from peripheral blood mononuclear cells at different time points after infusion of axi-cel. DdPCR was carried out to detect FMC63-28Z-1 amplicons as recently published by our group ( 13 ).…”

“…From a technical perspective, different methods have been used to monitor PB CAR T-cells in patients, based either on PCR [ 40 , 41 , 42 ] or flow-cytometry [ 43 , 44 ]. As compared to PCR-based approaches, flow-cytometry CD4 and CD8 CAR + subpopulations can be distinguished [ 44 ].…”

“…Other approaches that target T cell signaling sequences in quantitative PCR assays, real-time (qPCR) [ 16 ] or endpoint PCR (dPCR) [ 23 25 ] are suitable for CAR T cell quantification. Here, we designed our own ddPCR assays based on 2 experimental CARs with known sequences, which were validated and tested in murine experiments.…”
