Diffusion-weighted imaging versus contrast-enhanced MR imaging for the differentiation of renal oncocytomas and chromophobe renal cell carcinomas

Abstract: • ADC values can assist in differentiating renal oncocytomas from chromophobe RCCs. • DW imaging possesses better specificity than does contrast-enhanced MR imaging. • Combining the two parameters provides higher specificity regarding the differential diagnosis.

“…Another limitation is the lack of research on the value of contrast-enhanced MRI in this study. However, contrast-enhanced imaging might be inferior to DWI in renal tumor diagnosis, especially for complicated renal lesions, hemorrhagic lesions, or tumors showing overlap in the enhancement pattern (2836). Moreover, most patients with renal tumors are elderly individuals and may have renal dysfunction, which increases the risk of development of nephrogenic systemic fibrosis after contrast administration (37).…”

“…ER was calculated as follows: ER=(SI max – SI pre )/SI pre ×100%. 15 Wall thickness at the neck, body, and dome portions of the IAs was measured manually on the post-enhancement images. Average values were used for all subsequent statistical analyses.…”

“…The combination of DWI‐MRI and DCE‐MRI seems to provide a more accurate diagnostic method for differentiating ccRCCs from other less‐aggressive renal cortical tumors. Increased vascularity in malignant tumors can decrease diffusion and increase perfusion . Then, based on high vascularity and cellularity, IVIM can divide ccRCCs into four grades that are positively correlated with (perfusion fraction) f values and negatively linked with (diffusivity) D values and (pseudodiffusivity) D* values in DWI .…”
