Diffusion Tensor Tractography versus Volumetric Imaging in the Diagnosis of Behavioral Variant Frontotemporal Dementia

Abstract: MRI diffusion tensor imaging (DTI) studies of white matter integrity in behavioral variant frontotemporal dementia have consistently shown involvement of frontal and temporal white matter, corresponding to regional loss of cortical volume. Volumetric imaging has a suboptimal sensitivity as a diagnostic tool and thus we wanted to explore if DTI is a better method to discriminate patients and controls than volumetric imaging. We examined the anterior cingulum bundle in 14 patients with behavioral variant frontot… Show more

“…The diffusion anisotropy has been shown to correlate with the progression of a wide variety of conditions (Kubicki et al, 2002). For example, reduced FA is observed during ageing (Hsu et al, 2010; Sullivan and Pfefferbaum, 2006), and in neurodegenerative diseases such as dementia (Englund et al, 2004; Santillo et al, 2013), Parkinson’s disease (Surova et al, 2013), Alzheimer’s disease (Sjobeck et al, 2010), and multiple sclerosis (Rovaris et al, 2005). By contrast, the value of FA tends to increase during white matter (WM) maturation (Lebel et al, 2008; Löbel et al, 2009) and after specific forms of training, such as juggling (Scholz et al, 2009).…”

“…For bvFTD, measures of white matter disease are thought to be more sensitive than those for gray matter disease. In particular, two studies show that tractrography and DTI measures are more sensitive in terms of bvFTD diagnosis than gray matter volumetrics, indicating decreased fractional anisotropy (explained in the Methods section) in the cingulum, anterior corpus callosum, and uncinate fasciculus (Powers et al 2014; Santillo et al 2013). Other studies show that changes in white matter diffusion correlate with gray matter atrophy in bvFTD (Avants et al 2010; Borroni et al 2007; Whitwell et al 2010).…”
