Diffusion tensor tractography of the arcuate fasciculus in patients with brain tumors: Comparison between deterministic and probabilistic models

Abstract: Purpose The purpose of this study was to compare the deterministic and probabilistic tracking methods of diffusion tensor white matter fiber tractography in patients with brain tumors. Materials and Methods We identified 29 patients with left brain tumors <2 cm from the arcuate fasciculus who underwent pre-operative language fMRI and DTI. The arcuate fasciculus was reconstructed using a deterministic Fiber Assignment by Continuous Tracking (FACT) algorithm and a probabilistic method based on an extended Mont… Show more

“…23 Because brain stimulation techniques are less effective for WM tracts than for GM, evaluating the morphologic localization of the AF is essential for appropriate presurgical planning. [24][25][26][27] DTI-based tractographic studies revealed incomplete AF reconstructions, in particular of the anterior portion. 24 The evaluation of this AF segment is considered an important predictor of postoperative outcome because a lesion in the anterior portion of AF could cause negative effects on speech fluency.…”

“…For language tracts, grades also incorporated the anterior-most extension of the arcuate fasciculus towards Broca's area. (24,25) Readers also graded overall preference for the DTI and CS-DSI tracts as 0=no difference, 1=DTI superior and 2=CS-DSI superior.…”

“…Moreover, it is worthy to note that a pre-surgical planning performed by means of DTI-based tractography cannot provide any qualitative and quantitative information regarding well-known undetected white matter bundles, such as the lateral portion of CST as well as the anterior portion of AF. 29,32 All these limitations might be avoided by using CSD-based approach. Its sensitivity for reconstructing white matter bundles has been also recently highlighted by a comparative analysis of different tractographic approaches; in detail, CSD-based tractography was able to better detect brain pathways involved by neoplasm if compared to DTI-based one.…”
