Diffusion of OXA-48 carbapenemase among urinary isolates of Klebsiella pneumoniae in non-hospitalized elderly patients

Abstract: Background Recently, a dramatic increase of Klebsiella pneumoniae positive for OXA-48 β-lactamases was observed first in the hospital setting and later in the long-term care facilities (LTCFs) and community in the Zagreb County, particularly, in urinary isolates. The aim of the study was to analyse the epidemiology and the mechanisms of antibiotic resistance of OXA-48 carbapenemase producing K. pneumoniae strains isolated from urine of non-hospitalized elderly patients. … Show more

“…In general, ESBLs and KPC carbapenemase genes were the most detected ( Figure 4 ), confirming their dissemination in MDR K. pneumoniae isolates as verified in several other studies [ 33 , 40 , 41 , 42 , 43 , 44 , 45 , 46 ]. KPC enzymes were originally identified in K. pneumoniae which justifies its high prevalence in cases of resistance to carbapenems in this species.…”

“…These species are known to be good vectors for plasmids. There are a lot of strains positive for carbapenemases in our ICUs, as described in the previous studies [ 3 , 4 , 5 , 6 , 7 , 8 , 9 ]. It is possible that cross infections occurred between patients in the same hospital ward.…”

“…The dominant combination was OXA-48+NDM, which is in concordance with a multicenter study recently carried out in European countries. All combinations contained OXA-48, which is the most widespread type of carbapenemase in Croatia in the last decade, significantly outnumbering the KPC and VIM variants [ 7 , 8 , 9 , 32 ]. KPC was the additional carbapenemase detected in the isolates from Split, whereas those from Zagreb and Osijek contained MBL and OXA-48.…”

“…Klebsiella pneumoniae is recognized as a conditioned pathogen that colonizes the mucosal surface of patients and do not cause illness, but K. pneumoniae from the mucosa may spread to other tissues, leading to life-threatening infections and a variety of diseases, including pneumonia, urinary tract infections, blood infections, and sepsis [ 1 , 2 ]. These infections are particularly prominent in newborns, the elderly, and individuals with weakened immune functions [ 3 ]. K. pneumoniae also causes a large number of community-acquired infections.…”
