Diffusion of contrast medium after perineural injection of the palmar nerves: Anin vivoandin vitrostudy

Abstract: Significant proximal diffusion occurs in the first 10 min after perineural injection in the distal aspect of the limb and should be considered when interpreting nerve blocks. Distribution of local anaesthetic solution outside the fascia surrounding the neurovascular bundle or in lymphatic vessels may explain delayed or decreased effects.

“…We speculate that if contrast medium is injected more proximally, then it is more likely to diffuse along the neurovascular structures within the neurovascular bundle, as seen in a previous study where contrast medium was injected around the palmar nerves at the base of the proximal sesamoid bones (Nagy et al . ). The distal injection site in the current study was distal to the site where the neurovascular structures diverge, which may explain why the contrast medium did not appear in such a proximodistally elongated shape as after the proximal injections.…”

“…is not analgesic, and may also facilitate the recognition of low grade lameness because of a general reduction in muscle tone, and may be preferable. Inadvertent injection into lymph vessels may result in false negative responses to perineural analgesia (Nagy et al . ). If the clinical signs and response to local analgesia are inconsistent it may be worth repeating the block on a subsequent occasion. The clinician should be aware of the fact that in multiple locations, most of which are mentioned in this review, inadvertent penetration of synovial structures can occur during perineural injections.…”
