Objective: The relationship between apparent diffusion coefficient (ADC) and chemotherapy has been established. However, whether ADC could be considered as a measure for monitoring response to sorafenib in hepatocellular carcinoma (HCC) has not been demonstrated. This study was to investigate the ADC changes of advanced HCC under sorafenib treatment. Methods: Athymic mice with HepG2 xenografts were allocated to two groups: control and sorafenib (40 mg/kg, bid). T2 and diffusion images were acquired at each time point (0, 10, 14, and 18 d post-therapy). Tumor volume and changes in ADC were calculated. Results: Tumor volumes on Days 10, 14, and 18 after treatment showed significant decreases in the sorafenib-treated group compared with the control. Pretreatment ADC values were not significantly different between the control and treated groups. A slow increase in ADC in the peripheral zone of tumors appeared in the treated group, which was significantly higher compared with the control group on Days 10, 14, and 18. In the central part of tumors on Day 10 after treatment, an increase in ADC appeared in the treated and control groups, the ADC of the control group being significantly lower compared with the treated tumors. From Day 10 to Day 14, the ADC map showed a progressive decrease in the central region of tumors in the treated and control groups. However, this change is more significant in the treated groups. Conclusions: Early changes in mean ADC correlated with sorafenib treatment in HCC, which are promising indicators for predicting sorafenib response in this carcinoma.

N eoadjuvant chemoradiotheraphy (CRT) improves tumor downstaging, pathological complete response (pCR), and local control (1, 2). pCR rates of 13%-30% have been reported in phase II and phase III trials following 5-fluorouracil-based preoperative CRT (3, 4). Currently, management of patients with clinical complete response (cCR) remains controversial (5-8).A recent meta-analysis including 218 phase I/II or retrospective studies and 28 phase III trials of adjuvant CRT reported that T3 rectal cancer is associated with high local recurrence rates after nonsurgical treatment (9). In addition, similiar results were recently shown from a study using a "wait-and-see" policy after CRT (10).Accurate imaging methods are needed to evaluate CRT responses, and post-CRT magnetic resonance imaging (MRI) is frequently used for this purpose. However, the method has low accuracy in predicting the pathological stage of the tumor and can often overstage T1 and T2 tumors due to the limited capability of MRI to differentiate viable tumor, residual fibrotic nontumoral tissue, and a desmoplastic reaction. Understaging of irradiated rectal cancer can affect treatment planning, including the surgical strategy, and thus affects the tumor recurrence rate and prognosis (11).Diffusion-weighted (DW)-MRI is a functional imaging technique that yields qualitative and quantitative information and provides unique insights regarding tumor cellularity, integrity of cell membranes, and microcirculation.

The ability to noninvasively discriminate the most common types of renal cell carcinoma (RCC) based on their magnetic resonance imaging (MRI) appearances or their magnetic resonance phenotypes is achievable. Intracellular lipids, a histologic characteristic of the clear cell RCC, can be identified on chemical shift MRI. Intratumoral hemosiderin deposition, as observed histologically with papillary RCC, correlates to the low signal intensity on T2-weighted images. In addition to morphologic imaging features, the different subtypes of RCC have distinct patterns of enhancement on dynamic contrast-enhanced MRI. Clear cell tumors demonstrate much greater enhancement than papillary and chromophobe RCCs during the corticomedullary and nephrographic phases. The MRI technique arterial spin labeling (ASL) uses magnetic fields to label the water protons in arterial blood and measures blood flow into tissue; quantitative images of blood flow can be generated without exogenous contrast media. Multiple measurements of tumor perfusion may be repeated before and after a physiologic or drug challenge (ie, antiangiogenic therapy). In RCC xenografts and in patients with metastatic RCC, ASL MRI assessments can be used to monitor blood flow changes in response to antiangiogenic therapies. High blood flow on ASL MRI in RCC xenografts correlates with viable tumor at histopathologic analysis, whereas zones of absent or diminished signal correspond to necrosis. ASL MRI has the potential to serve as a biomarker for patients with metastatic RCC by offering a noninvasive measure of tumor blood flow changes accompanying antiangiogenic therapy.
