PurposeTo describe the ophthalmoscopic features and natural history in a case series of eyes that developed intraocular hemorrhages associated with perinatal distress and to evaluate their clinical courses.MethodsA retrospective chart review of 289 neonates with a medical history of perinatal distress was conducted. Among these 289 patients (578 eyes), 29 eyes of 17 neonates were found to have had retinal hemorrhages or vitreous hemorrhages (VH). A comprehensive chart review, including details of fundoscopic findings and perinatal history, was conducted.ResultsIntraocular hemorrhage was present in 5.5% of the patients. Most hemorrhages (82.7%) were intraretinal. In our population, 17% (n = 5) of hemorrhages resolved within two weeks, but 31% (n = 9) did not resolve even after four weeks. Most hemorrhages spontaneously resolved without any specific sequelae; however, one infant's dense unilateral VH persisted up to three months after birth. When the patient was seen again at 3.5 years of age, she had developed axial myopia and severe amblyopia of the involved eye.ConclusionsIn asphyxiated newborns, the possibility of intraocular hemorrhages should be considered. Long-standing, dense hemorrhages obscuring the macula may lead to severe vision deprivation amblyopia. Therefore, ophthalmic examination should be considered in neonates with perinatal distress, and close observation is necessary for hemorrhages that do not resolve in this amblyogenic age group.

Московский научно-исследовательский институт глазных болезней им. Гельмгольца, Москва, Российская Федерация Ретинальные кровоизлияния у младенцев являются частой патологией, однако причины, клинические формы и функциональные исходы исследованы недостаточно. Особенный интерес представляет изучение локализации кровоизлияний в структуре глаза, сроки их рассасывания и отдаленные последствия, которые могут влиять на развитие зрения. Цель исследования -анализ клинических форм, сроков рассасывания и отдаленных клинико-функциональных исходов ретинальных кровоизлияний у младенцев. Методы. Все дети с ретинальными кровоизлияниями в возрасте от 3 нед до 3 мес, обратившиеся за помощью в кабинет недоношенного ребенка МНИИ глазных болезней им. Гельмгольца в период 2011-2015 гг., обследованы с использованием метода непрямой обратной офтальмоскопии. При обширных поражениях контроль динамики процесса выполняли на цифровой ретинальной педиатрической камере. Отдаленные клинико-функциональные исходы ретинальных геморрагий изучены посредством ультразвукового исследования и спектральной оптической когерентной томографии. Результаты. Кровоизлияния на глазном дне выявлены у 108 (5,9%) из 1825 младенцев на 142 глазах (у 34 детей были двусторонние кровоизлияния). Геморрагии чаще отмечены у детей, рожденных через естественные родовые пути (79/108 детей; 73,2%), были односторонними (74/108; 68,5%), преретинальными (108/142 глаз; 76,1%), центральной локализации (119/142; 83,8%). Сроки рассасывания кровоизлияний у 53/108 (49,1%) детей составили более 1 мес. Отдаленные исходы ретинальных кровоизлияний изучены у 22 детей (33 глаза) в возрасте 2-5 лет. Остаточные изменения в структуре нейроэпителия и витреоретинального интерфейса со снижением остроты зрения отмечены у 7 детей (9 глаз). Заключение. Ретинальные кровоизлияния новорожденных отличаются клиническим полиморфизмом, разными сроками рассасывания и различными исходами, что требует динамического наблюдения. Post-mortem intraocular findings in two autopsy cases with traumatic intracranial haemorrhage were obtained using an ophthalmological endoscope. The endoscopy results clearly revealed the presence of intraocular haemorrhages and papilledema caused by intracranial haemorrhage. Post-mortem ophthalmological endoscopy offers several benefits. First, post-mortem intraocular findings can be directly observed in corpses with post-mortem clouding of the cornea. Secondly, the endoscopy only requires a 0.9 mm incision in the sclera and does not require the removal of the eye from the corpse, a procedure that should be avoided for ethical and cosmetic reasons. Thus, post-mortem opthalmological endoscopy is a useful method for obtaining intraocular findings in autopsies.

Abstract: Although retinal findings in nonaccidental trauma are common, accidental strangulation retinopathy in neonates is a rare occurrence. To the authors' knowledge, this case is the only documented histopathologic study of hemorrhagic retinopathy associated with strangulation in the literature.
