Diffuse Subamniotic Calcification: A Novel Pattern of Placental Calcification

Abstract: Two primary patterns of placental calcification have been described, each with distinctive pathophysiology and clinical relevance. We report a novel pattern of diffuse subamniotic calcification. It occurred in a 25-week placenta involved by recurrent chronic histiocytic intervillositis (CHI) associated with severe intrauterine growth restriction (IUGR) and intrauterine fetal demise (IUFD). This was the mother’s third stillbirth related to CHI, despite treatment with intravenous immunoglobulin (IVIG), prednison… Show more

“…Notably, in Case 3, despite a reduction in CHI severity and C4d staining in the placenta following immunomodulatory therapy, the pregnancy unfortunately resulted in a stillbirth at 25 weeks GA, and abnormal histologic findings were noted in the post-therapy placenta, which had been previously reported by our colleagues. 23 Specifically, the placenta showed diffuse subamniotic calcifications, which were highly suggestive of immune complex deposition based on electron microscopy. Our colleagues hypothesized that IVIG treatment may have contributed to immune complex deposition and subsequent dystrophic calcifications in this placenta.…”
