Diffuse Sclerosing Variant of Papillary Thyroid Carcinoma

Abstract: The purpose of this pictorial essay is to show the limitations of sonography and complementary usefulness of specimen radiography in detecting microcalcifications of the diffuse sclerosing variant of papillary thyroid carcinoma, which mostly manifests as diffusely scattered microcalcifications in the thyroid gland.

“…Incalculable nodule-independent microcalcifications A large number of incalculable nodule-independent microcalcifications that are typically snowstorm-like are diffusely scattered in the whole or large area of unilateral or bilateral thyroid gland, which is always seen in the diffuse sclerosing variant of PTC [142,143]. If typical cervical metastatic lymph nodes of thyroid cancer are observed, these findings are assessed to belong to C-TIRADS category 5 (Fig.…”

“…Solid and predominantly solid nodules are also more likely to be malignant compared to cystic nodules [32]. It is worth noting that the diffuse sclerosing variant of PTC may present as diffusely infiltrating disease resulting not in a single nodule but in diffuse enlargement of a lobe or the entire gland [33,34]. Diffuse thyroid enlargement, especially if accompanied by cervical lymphadenopathy, must therefore prompt imaging for the microcalcifications that are invariably associated with this subtype [33,35].…”

“…If dots are present in both, they are probably not PEF for the purpose of ACR TI-RADS classification. The only exception is the diffuse sclerosing variant of papillary carcinoma, which should not present a problem in diagnosis (13).…”
