Diffuse increased splenic F-18 fluorodeoxyglucose uptake may be an indirect sign of acute pyogenic cause rather than tuberculous in patients with infectious spondylitis

Abstract: On the basis of the results presented, this study demonstrated that some quantitative indices from F-18 FDG positron emission tomography/computed tomography images could be indirect signs of an acute pyogenic cause of the IS. Among the various quantitative indices, spleen SUVmax, S/B ratio, and S/L ratio were potent indicators for an acute pyogenic cause of the IS.

“…As a specialized immune organ, the spleen has various functions, such as clearance of microorganisms, the site of development for lymphocytes (both T and B), release of immunoglobulins, and production of immune mediators 31 . Generally, 18 F-FDG uptake is related to tissue metabolism, which may explain why an increased 18 F-FDG uptake in the spleen may reflect increased glucose consumption in the spleen in the event of an infection 21 . A recent study demonstrated that current inflammation could result in diffuse splenic 18 F-FDG uptake 32 .…”

“…Diffuse splenic uptake is a visual observation on PET/CT corresponding to a global capture of 18 FDG by the organ, relative to liver uptake. This phenomenon, described and used as a potential severity criterion in oncology (11)(12)(13)(14), hardly referred in pyogenic infections (15), has never been reported in IE. In this prospective study, this phenomenon was observed in 24 patients, among them 23 IE (resulting in a specificity of 96%).…”
